Firstly apologies for writing in French. The main points below are concerning:-

To get a wider audience how to convert into English. 

Looking through any sleep report to pick out items that may be germane to his therapy.

Juste quelques mots en français afin de peut-être faciliter les choses:-

D'abord ce sera intéressant de présenter vos graphiques en anglais. 
Normalement, si vous allez en “Aide/changer de langue”…. il y a un choix des langues, ensuite, sélectionnez “Anglais”. (Vous pouvez facilement revenir en arrière après.) 

Personnellement, j'utilise une machine Resmed Autosense, donc je n'ai pas de connaissance de votre machine. Bien sûr, il y a des utilisateurs qui ont le même de la vôtre et vos graphiques en anglais seront intéressants afin de vous aider. 

Dernière chose, est-ce que vous avez eu une polygraphie ventilatoire (home test) ou une polysomnographie ?  

Souvent, on trouve des indices très intéressants, par exemple, analyse des événements respiratoires entre IAH dorsal, IAH non dorsal, apnées obstructives, apnées centrales, saturation en oxygène….. 

Donc bon courage, les gens ici sont formidables pour vous aider.

merci pour vos conseils mais l'aide changer de langue ne me donne aucune possibilité.J'ai fait une polysomnographie qui a révéle 20 AIH  
50%position dos.
j'ai vu récemment mon pneumologue,sur le papier tout va bien:AIHmaitrisé donc c'est bon.Ce n'est pas mon ressenti.
J'ai pas mal de fuites avec mon masque resmed N 30i.J'en ai essayé une dizaine,niveau fuite c'est pareil quelque soit le masque mais avec celui là les fuites ne sont pas dirigées vers les yeux et il n'est pas encombrant.

I recently saw my pulmonologist, on paper everything is fine: AIH mastered so it's good. That's not my feeling.
I have a lot of leaks with my resmed N 30i mask. I have tried around ten, the leak level is the same whatever the mask but with this one the leaks are not directed towards the eyes and it is not not bulky.

Je vous remercie pour votre réponse. 

A propos du changement de langue, c’est vrai, j'ai oublié que depuis v1.5, cette option ne marche plus. On m’a dit que c'était prévu que ça fonctionnerait à nouveau dans le courant de ce mois-ci. 

Concernant votre expérience avec le pneumologue, un chiffre à moins de 5 IAH,  “pas de problème, vous êtes guéri circulez, il n'y a rien à voir”….! 
Malheureusement typique,,,, partout.

En plus, le calcul d'IAH ne prend pas en compte la durée de chaque événement. 
Par exemple une apnée obstructive qui dure 60 secondes, et une  autre qui dure que 20 secondes, ou une hypopnée qui dure 10 secondes. D’autre part, on ne parle jamais des “micro-réveils” qui ne sont pas chiffrés dans le calcul d’IAH. Ces micro-réveils sont très perturbateurs durant la nuit, et impactent la qualité du sommeil. 

Il est très possible aussi que votre IAH soit inexacte à cause de vos fuites. 
Donc difficile d'établir une titration bien adaptée de vos propres circonstances. 

Le problème des fuites est très important, et à solutionner, dès que possible. 

Les fuites sont normalement, soit dues à l’étanchéité, soit buccales, surtout si vous utilisez un masque narinaire ou nasal. 

D'après moi, il est peu probable que l'étanchéité de votre narinaire soit en cause. C’est peut-être des fuites buccales,  Durant la période du sommeil paradoxal, il y une sorte de “paralysie du sommeil”. Les muscles perdent leur tonicité, le menton peut descendre, la bouche s'ouvre…….

Les solutions pour éviter ces fuites buccales sont diverses, par exemple le sparadrap pour fermer la bouche ou également une mentonnière. Une autre possibilité est un collier en éponge/mousse. (Soft cervical collar). 

Personnellement j'utilise le sparadrap.

The problem of leaks is very important, and needs to be resolved as soon as possible.

Leaks are normally either due to mask seal, or mouth leakage, especially if you use a pillow or nasal mask.

In my opinion, it is unlikely that your mask is to blame. More likely mouth leakage. 

This can occur during the REM, when there is a sort of “sleep paralysis”. The muscles lose their tone, the chin can drop, the mouth opens…….

There are various solutions to prevent this situation, amongst these for example mouth tape,chin strap, or another possibility is a sponge/foam collar. (Soft cervical collar).

Personally I use mouth tape.. .

I hope this information helps a bit.

I suggest going back to the settings you used on 16/12/2023 as that has yielded the best results. It looks like it was a pressure of 6-10 no SoftPAP.

What does the rRMV curve represent?

I merged your latest post with your previous thread.

I believe most of your issues lie with the high leak rate, which you haven't been able to resolve yet. A high leak rate can affect your cpaps ability to maintain proper air flow and can affect therapy. So try to figure out if it's a problem with mask fit or you are mouth breathing.

Most here are not too familiar with your machine.

If you would order the Clinicians Manual, it may explain what rRMV represents.
