Three questions: (1) Has the need for therapy changed and the old machine does not do that (ie - change from CPAP to BiPAP)? (2) What does the new machine do better than the old one and are those things really important to your treatment? (3) Is the machine failing mechanically?
Now, there is a point a machine reaches which will dictate replacement: technical obsolescence (no support (parts, repair, technicial support)) or mechanical deficiency.
