DISCUSSION:
This study was performed to assess for the optimal positive pressure therapy levels for treating the patient's sleep-related breathing disturbance the night prior to performing a MSLT. Various pressures were utilized. The pressures attempted during this study ranged from a low of 16 cm of H2O to a high of 22 cm of H2O. The patient did best on Spontaneous Bi Level with pressures of IPAP 21 cm and EPAP 16 cm with Trigger sensitivity set to High using a Mirage FX nasal Standard sice mask with the addition of a chinstrap, XL Size.   The chin strap was added due to oral pressure leak. We used his oral dental appliance the night of the recording.
Severe Periodic Limb Movements of Sleep (PLMS) were identified, which could be an additional factor leading to sleep fragmentation and may warrant consideration for treatment. The PMS index was 38 / hr of sleep.
Video EEG was recorded simultaneously with the polysomnogram to evaluate for possible seizure activity. A full video EEG report will follow analyzing the electrographic data.
The patient had a Multiple Sleep Latency Test the following day, the results of which are reported separately.  Please call with any questions regarding the interpretation of this study.
