Waking up a couple hours early and can't get back to sleep - JiveMiguel - 11-26-2015

Hello all!
I'm a new member here, but I've been using my machine for about four years. I've had very little problems with it and took to it almost immediately.

But for the last couple days, I've had a new issue: I wake up around 5:30 am for some reason, and can't get back to sleep. I sense that I'm dozing back to sleep, but then I wake up again, breathing heavily. This happens three times maybe, then it's time to get up. I'm losing about 1.5 to 2 hours of sleep because of this. I go to bed around 11pm. I have no problem getting to sleep initially.

I don't know if my machine is a CPAP or a BiPAP or ???. My machine specs are in my profile. I've gone into the clinician menu and upped my pressure from 8.5 to 9.5 but that doesn't help. But I admit that I'm shooting in the dark since I don't understand most of the clinician menu items.

Can anyone give me some guidance on what to adjust or look for in the clinician menu or why this is happening at all?

I'd also like to know if I can use any kind of software with my machine.

Welcome to ApneaBoard!

Perhaps your body has just had enough sleep when you first awaken? Not sure what the waking with heavy breathing is all about.

Maybe it's time for a new sleep study?

The DeVilbiss Intellipap Standard Plus is a straight CPAP machine with EPR.
It has Auto-Start. Any chance you're machine has stopped running at 5:30 AM?
It might do that if you have a large leak or a failed part like the hose.

I believe Sleepyhead software will work with you machine. It's a free download. See top of page.
I would think that after 4 years, it's time to take control of your therapy.

If you start looking at your data to see what the breakdown of your AHI is, and this way, we may be able to help you with a pressure setting.

It's also possible after 4 years that the machine is not working properly, or you may need a new sleep study. So let's start with the sleepyhead Download and go from there.

What wakes me up around 5:30 is usually something like my wife moving around or a cat jumping up on the bed. I checked for leaks and failed parts/hose and everything looks fine.

As far as I can tell, my DeVilbiss doesn't have a SD card slot. It has what looks like a PS2 connector (old style keyboard and mouse connections), but I don't know how that can be used.

It is not compatible with available software.

Thanks. I didn't think it was compatible.

I've just changed the SmartFlex setting. It was at 3 which gives the least resistance while exhaling. It seems to me that the problem is on exhale, I don't inhale again (an apnea event?), and that's why I wake up breathing heavily. So a lower setting increases the resistance (amount of air being pushed into the mask) during exhale. This might then trigger me to inhale and thus solve the problem. I set it to 2 and will see what happens tonight. It can also be set to 1 which creates even more resistance during exhale. But I'll try it one step at a time.

After changing the setting I mentioned above from 3 to 2, I slept better last night. I did wake up a couple times, but in all cases I went back to sleep within a few minutes and stayed asleep. Hopefully that will continue.

Thanks for this. I forgot that this machine does the smart codes. I retrieved them today and entered them into the report generator and got the following information. Maybe someone could tell me if these are good scores or not. They seem pretty positive to me:

AHI - Apnea/Hypopnea Index (AHI) represents the average number of apneas and hypopneas per hour for the timeframe.
last 90 days - 3
last 30 days - 3
last 7 days - 3.5
last day - 2.5

High Leak Flow Time
Shows the percentage of time where leak was above 95 liters per minute. A value of 10% or more indicates that the mask fit should be checked.

all time frames - 0

NRI
Non-Responding Event Index (NRI) represents the average number of non-responding events per hour. A non-responding event is a respiratory event that is detected but by design does not result in a pressure change during AutoAdjust therapy.

last 90 days - 1.25
last 30 days - 1.5
last 7 days - 1.5
last day - .75

EPI
The Exhale Puff Index (EPI) is the number of exhale puff events detected per hour. Exhale puffing is caused by inhaling through the patient circuit and exhaling through the mouth.

These numbers all seem pretty good to me. Can anyone give me any input?
