Question on ASV backup rate - Snowleopard28 - 12-04-2015

Hi everyone,
I was hoping someone could tell me if the backup rate on the PR System One BiPAP AutoSV Advanced machine is set to "Auto" will it give a breath if I stop breathing because of a central event? My sleep doctor told me that it won't. The only way this machine will ventilate if you stop breathing due to central events, is if the backup rate is set to a specific rate.
I'm asking because he had it set at 12, but I have asthma and sometimes it can take me longer to exhale. With the backup rate set at 12, the machine would try to push a breath when I was still exhaling. So I changed it to Auto. My doctor understood but he was worried.
To me, it doesn't make sense. The machine is in ASV mode and whether it's set to 12 or Auto, it should activate when there is no flow.

I don't know the answer for certain, but your doctor may have been thinking about the BiPAP/ ST which has a timed breath. I believe the Auto SV uses an algorithm based on an average of preceding breaths.

You can request the clinical manual which will give you many of the technical answers by following the link at the top of the page to CPAP Setup Manuals.

I've looked through the clinician's manual and couldn't find anything. But I thought the same thing as Sleeprider about the algorithm. My sleep doctor didn't tell me to switch it back to 12 bpm, so I left it on Auto. I'm not sure what to do. When set to 12, the machine and I are out of sync, but when set on Auto it's much better. The only thing that worries me is if my doctor is correct, and I have a run of central events or stop breathing, the machine will not do anything. It's confusing.

The auto setting has worked for most people I know that use that machine. It seems natural and comfortable. I'm pretty sure that it won't leave you stranded for a breath. Again, since it uses an algorithm based on YOUR breathing pattern, to maintain that pattern, it should seem pretty seamless.

This webpage confirms when Rate is set to automatic that central apneas are still treated. However, it is a supplier link, and we will need to copy it into our browser's address bar, since functional links to suppliers are not allowed on Apnea Board.

I have my backup in auto and it breathes fine for me when it needs to. I also found something interesting when I switched to the ASV several years ago - I was more comfortable setting the ex-hale relief at 1, when before using the A-flex or C-Flex C-Pap machines I needed the relief set at 3. The difference was that I was now using Bi-Pap and ASV in auto breath.

Thanks for all the info everyone. My doctor had me do a overnight oximetry while using my machine on that setting. I was sent the results electronically. It said that the results on AUTO BiPAP were normal. I have no idea what that means except that my sats were fine. Do you think he's talking about a different machine?

Hi Snowleopard28,

I think your doctor is talking about your [ASV] BiPAP machine when its backup rate is set to Auto, and means that he thinks the Auto setting for backup rate is working well for you.
