Good numbers, still can't sleep. - Rcgop - 06-11-2016

All numbers are very good but I still can't sleep.

Best I do is 1 hour at a time for the first half of the night. Then mostly awake the rest of the night. First half AHI is around 0 to 1, then last half of night AHI 4 to 6 for an average AHI around 3.

Any suggestions?

Hi Rcgop. It sounds like you've got some serious insomnia going on. Was this something that started with CPAP use, or has it been going on longer than that?

It would help the folks in the forum to advise you if they could see the daily graphs. Someone may be able to identify what is waking you up.

If you have not set up sleepy head there is a link at he top of the page just under the Apnea Board address.

The two things that most often cause loss of sleep with folks starting positive airway pressure, are leaks and high pressure, creating difficulty exhaling. We acclimate to the higher pressure overtime, it can help to start lower and increase the pressure gradually.

The leaks need to be minimized to have successful therapy.
You also could try dropping the high setting to 15 to see if that helps, but if you do watch for any elevation in you AHI.

I also have higher numbers in the second half of the night

Screen shot from sample day. What wakes me up every hour or keeps awake all night. Leaks are in line. Pressure is not too high. It does change which might be part of it. The constant waking was the main reason for seeing the sleep Dr. in the first place.

Fighting some of the same here. Numbers fine means that the therapy settings are correct for you. IMHO what is happening is that some of the comfort settings are "off" for you and you are not comfortable to sleep thru the night. I was at 30-60 minutes and have slowly gotten quite a bit better. The problem is that some of the changes I made are only available on the AirCurve VPAP (BiLevel) machine. Still not sleeping thru the night, but with the suggestions from some here am making progress again.

I am very sensitive to exhale pressure and when the machine detects the start of my exhale. Any interruption in my exhale disrupts my sleep. This of course does not mean that exhale pressure is what is impacting you, but something is.

I suggest you start making adjustments to the different comfort settings and keeping a journal so you remember what works and what does not. If you still can't make enough of an impact, you may need to change to the AIrCurve and gain that extra control and exhale range.

Hi Rcgop,

I suggest your Min Pressure is too low. I suggest walking it slowly up to around 7, taking a couple weeks to walk it up.

Might not help much, but I think it will help.

Moved min up to 6 with very good results. I am thinking about raising it to seven except I have started to sleep for a few hours. My AHs and OAs have dropped to almost 0. I think my problem now is the wake/sleep junk at the end of the night.

Will work on the comfort settings for the next week.
