AHI out of control - rmclendon - 05-24-2017

I am a very long term Apnea patient (on my seventh machine, >35 years). About six to eight months ago my AHI began to go up to the 40-55 range. I just had a sleep study that resulted in going to a Resmed AirCurve 10 ST. Prescribed pressures are 10 and 16.

In the last week the range has been 3.8 to 45.

Any thoughts or suggestions are appreciated.

IIf this was happening to me, I would insist on getting a copy of the sleep study so that you can review. You can then post to this site so that expert members can reach out to you to help you understand your prescribed treatment and hellp you evaluate any other possible choices. Have you recieved your ST machine yet? I see that you have Sleepyhead on your profile, certanly get that going and post the graphs as well. I have started my treatment three months ago and witht this forums advice I have had great results. I think you need to be proactive and understand what, why and how to manage your treatment.

Yes, I have been using ST since mid March. I will run some graphs and post them. I'm having some computer issues so it may be after I replace my motherboard before I can do that.

My pulmonologist/sleep doc does not seem interested in helping me. Unfortunately other choices are limited in this aea.

If your not sure how to organize and post your SleepyHead data see the links below. It's important that the data be organized as described in the link so we will have the data necessary to assist you. I'm not an expert on the Aircurve but we have several members who are.

Hope you get your computer issue resolved.

Rmclendon, your case is unfortunately not unusual, and the prescription by your doctor was equally unfortunate.  The machine you need to resolve your central and obstructive apnea is call a ASV (adaptive servo ventilator).  It uses a smarter approach to resolve these problems.  While your ST is capable of providing fixed EPAP and IPAP pressure and has a timed backup, that won't solve complex apnea in most people.  The ASV is capable of using automatic varying EPAP to resolve obstructive events, and uses pressure support from 0 to 15 cm to address central events.  The Resmed Aircurve 10 ASV is particularly good, and uses pacewave technology that matches the rate and volume of your spontaneous breathing to set the rate for for induced breaths.  We have many members in the same boat. There are many more.  What you need to understand is that you are probably using the wrong machine, and your continued high rate of events demonstrates the failure of the ST technology.  It can't be made much better.  You need to ask your doctor why you were not evaluated for ASV.  Once you find the answer to that, we can help you to form a plan to improve your treatment; we just need to know if the doctor is on your side or not.

I just finished reading another thread where members encouraged the poster to fire his sleep doc and get a new one which he did and now has things under control.
Almost suspect any doc an say yep I am a sleep doc after reading some Wikipedia articles and maybe signing on with some sleep association to read their slick magazine/journal then can hang a shingle saying board certified member.
Am I cynical??? Huh

I would suggest rmclendon check with university teaching hospitals to see if they have a sleep clinic or recommend somewhere. Might be worth a day trip to go there with sleep study papers in hand (assuming you can pry them out of uninterested sleep doc hands).
