Effectiveness of adding EERS. Data attached. - foxfire - 09-24-2018

I sent an older relative of mine to see my sleep specialist in Boston. His former sleep specialist, although well intentioned, couldn't get a handle on the correct therapy to treat my relative due to a variety of comorbidities. After a sleep study it was decided he should stay with his current machine and pressures and just add 150mL or EERS. Here are the results.

Really impressive! Since you posted your own story about EERS I have suggested it as a possible solution several times. The problem is that it's not easy to source a pre-made EERS and no one out there seems to offer a fabricated solution. As a result you have to be kind of handy.

Whats EERS, it's the first time I'm hearing of it.

Enhanced Expiratory Rebreathing Space (EERS)

There are a number of journal articles you can find online by searching. We had a conversation about it with designs for building the EERS on Apnea Board earlier this summer. http://www.apneaboard.com/forums/Thread-Switched-from-ASV-to-CPAP-finally-getting-some-sleep?pid=265472#pid265472

Trying to understand just where EERS benefit comes into play I need to ask:  Neither your before or after examples show any Flow Limitation, both are 00.  Soo.. I would gather EERS needs a very low or zero Flow Limitation to work its magic?  Apparently EERS will not lower a high FlowLimitation ?..

I have drafted an initial article here http://www.apneaboard.com/wiki/index.php?title=Enhanced_Expiratory_Rebreathing_Space_(EERS)

Please consider this preliminary, and feel free to feed-back any ideas or corrections.

zzzzorro, the benefit of EERS is that it increases the volume of rebreathed air which can remedy therapy onset central apnea and hypopnea due to hypocapnea (lack of CO2) that occurs in a fully vented CPAP/BPAP circuit. It is not a correction for primary central apnea. In the example posted by Firefox, we can see a significant reduction of all apnea and hypopnea events upon installation of an EERS circuit. In this case, the person was experiencing 20 to 40 AHI and is now less than 1 AHI. An amazing turnaround. We can't promise that to be typical, but these results do have a sound foundation in respiratory theory and in limited reported results.

Wow, does this mean that if i block the front hole in my philips dreamwear it directly transforms into an EERS headset, since the vents are also at the top of your head away from the nose?

It would be great to see a few more before and after data, from other users who installed EERS.

The Whisper Swivel II Exhalation Port used in the EERS setup has a known intentional leak rate. You have no idea how much the leak rate would be on the dreamwear if you block the front hole.

I could always model the top vents in CAD and do a flow analysis with Solidworks. Is there a database on EERS, leak range etc. Where do you guys get the theoretical models to make these prototypes? I understand the basic idea but I'd love some hard data and numbers.

I mean you added 150mL, why not 100 or 200, did you use a captograph during the night to measure CO2 levels and then decided on 150mL?
