What does the 'EPR Inhale' setting do? - StevesSp - 07-04-2019
I've had my Resmed S9 Elite for a few years, yet only noticed this setting yesterday ...
In the clinician's menu, there's a setting labelled 'EPR Inhale'. I can choose between medium and fast (or something like that). It was set to medium, so I tried the other setting. I felt a difference, which I would describe as faster shifting from exhale back to presssure delivery, or perhaps a stronger initial pressure recovery (less ramp).
What is this setting for and does it have any significant effect on the efficacy of therapy (fixed pressure machine)?

No doubt this is referred to somewhere in the clinician's manual. I have copies of most of the Resmed manuals but not that one, so I can't help you directly. However go to the link CPAP Setup Manuals at the top of this page and you will be able to request a copy. Be sure to follow the instructions carefully as there is an enormous number of manuals available.

The S9 Elite and Autoset allow the user to set EPR and in CPAP mode you can set EPR Inhale to Medium or Fast as described on page 4 of the manual:
EPR checklist
- You can set EPR to Off, Ramp Only, Full Time.
- You can select an EPR Level of Patient (configurable by the patient), Off, 1, 2, or 3 cm H2O.
- In CPAP mode you can set the EPR Inhale to Med or Fast.
- When EPR is enabled, the delivered pressure will not drop below a minimum pressure of 4 cm H2O, regardless of the settings
The inhale setting refers to rise time, or the milliseconds that it takes for pressure to transition from EPAP to IPAP. In the S10 series and in Autoset mode on the S9, this is replaced by the use of Easybreathe™ which is a comfort feature that gives a wave-shaped form to the mask pressure. The image below shows the respiration flow rate and mask pressure wave form with Easybreathe. Compare to yours.

Technically EPR is a comfort setting, and the settings appear under comfort and are often made available on the patient menu. Because it works like a limited bilevel, it has a therapeutic application as well, and has the potential to cause a reduction in pressure that could increase obstructive apnea, especially in fixed pressure machines or individuals near their therapeutic threshold. That's my take.
It's interesting that in the Airsense 10 series, the Elite model does not have the return to inhale speed, suggesting all machines now use Easybreathe, however, it is possible to turn Easybreathe off in the VPAP-S mode on the Aircurve 10 bilevel machines, and a rise time setting is available in that option.

Thanks again. After years on the 'Medium' setting, I found the 'Fast' setting bothersome. No problem leaving it where it is. Just curious.
