Need help choosing right machine for OSA: Resmed Airsense VAuto or ASV - strasden - 07-07-2019
Hi!
I want to purchase a new PAP machine for my father, he currently uses ResMed AirSense 10 AutoSet and I was thinking to get something better for him. He has an Obstructive sleep apnea.
I was looking at resmed Aircurve series and wanted to get the best machine for his type of apnea. There are two models that I'm thinking about: VAuto and ASV. Which one of them will be better? Does it make sense to get ASV if he doesn't have central apnea, or is it the best machine for all kinds of apnea? Thank you in advance!

G'day strasden. Welcome to Apnea Board.
If your father just has "plain vanilla" obstructive apnea, then the Resmed Autoset is the best machine you can buy. If he has one of these and not getting good results, it may need adjustments to the pressure or other settings.
The VAuto is also used for obstructive apnea but is normally only indicated if the pressure is too high to be comfortable or if there is some related condition.
The AirCurve 10 VAuto is an auto-adjusting bilevel machine that uses the comfort of both the AutoSet™ algorithm and Easy-Breathe waveform in its VAuto algorithm to treat obstructive sleep apnea patients who can benefit from greater pressure support.
The ASV is only used for central / complex apnea and / or periodic breathing. It's not suitable for simple obstructive apnea.
The AirCurve 10 ASV bilevel machine offers truly personalized therapy for central breathing disorders, such as Cheyne-Stokes respiration (CSR), central sleep apnea (CSA) and associated obstructive events.
Can you tell us if your father has problems such as asthma, COPD (chronic obstructive pulmonary disease), heart condition or others? If he has none of these, then the Autoset should be entirely capable. If he's not getting good results with the Autoset then it's best to download and install the OSCAR software and post some daily pages here and we'll see if there are adjustments needed.

for just OSA get VAuto, it has everthing Autoset has + awesome comfort settings, you can match it to your fathers breathing pattern.
If you really wanted you could 'build' a new VAuto for $1000, eg. Get a new Autoset and a really used up VAuto and just take the motherboard out of VAuto and replace it into the Autoset. only 3 cables. Alternatively you can get a used up VAuto and get a new or close to new blower motor, although you can't check how much a blower is used. Only indication would be the color of the silicone seal, the more on the yellow scale the older the motor is, but that only indicates the age and not the usage though.

The Aircurve 10 ASV is used for complex and central apnea and is an inappropriate choice from what we know. The Aircurve 10 Vauto is an upgrade to the Airsense 10 Autoset, and has a greater range of pressure support (difference between inhale and exhale pressure) which can target flow limitations and hypopnea more effectively. The Vauto also allows subtle changes in the minimum and maximum times of inspiration and in the sensitivity to switching to inhale and exhale pressure. It is the best machine for treating obstructive sleep apnea, but due to its higher cost is not the primary choice for insurance reimbursement which views CPAP as good enough. The Airsense 10 Autoset is the closest thing you can buy to a bilevel machine without spending the larger amounts of money for a full-featured bilevel like the Vauto.
