Using Auto CPAP for 50 days - do not feel different and still tired? - rodimus82 - 10-05-2019

Hi, i am new to this and have been using cpap for just about 2 months.  i have a sleep study with AHI result above 30 (1 night only test using the Philips Alice).  After the results I was recommended by the ENT specialist to start on CPAP.  My results are decent with AHI below 5 at all times but i'm not sure if it works for me.  I dont feel tht i'm liberated and i'm don't feel much of a difference.  anyone experience the same feeling as i do?

Note that i breathe through my mouth as well and got a full face mask.  Does the machine work if i breath through my mouth?

I'm somewhat new to apnea board, so I will not attempt to analyze your charts. Someone else will have a look and advise. One thing that has helped me is to wear a soft cervical collar while sleeping. Yes, if you are wearing a full-face mask, the machine works if you breathe through your mouth or your nose.

Can you post your sleep study results as well? The reason I'm asking is because most doctors only care about your AHI but the important number in my opinion is your RDI and I want to see if there is a big difference between the two numbers.

By the way, how tall are you? I want to see if your mediam tidal volume is still low. It gives us a good indication if you are still dealing with flow limitations or not.

I'm also pretty new, but will note two things:
  1. your leak rate is pretty high
  2. your minimum pressure is pretty low
Normal recommendation on 2 is to raise the min by at least the same value as your EPR. So if your EPR is set at 2, you'd want at least 6. But looks like it should be higher still.

Guessing if you address those two things, it will help, but others with more experience can confirm and maybe offer more specific recommendations.

I'd consider a pressure change from the low of 4 to 6 or maybe higher. Unless specific reasons contradict this, most adults will feel starved for air at 4. This could be contributing to not feeling better for the therapy.

One thing for sure, your minimum pressure is way too low. I will raise it at least to 7 or 8. Will lower your AHI even further.
