CA events and overall thoughts on improvements - clroach - 02-12-2020

Last 3 nights of data.  I made changes to machine on last 2 nights Bonjour had suggested when I switched from CPAP to APAP.  Mainly bringing the minimum up from 4 to 7, and putting the EPR at 2.  I've made those changes and still have quite a few CA events.  Only been on therapy for 3 weeks and looking at Oscar data for 1.  Any thoughts or suggestions.  I'm definitely waking up 2 to 3 times a night.  Thanks

One of the issues with EPR is that in some individuals it can increase CA events.
Decrease your EPR from 2 to 1 and let's see if this is the case with you.

Increasing EPR or PS on BiLevel machines will treat (reduce) hypopnea, flow limitations, RERA, UARS, and snoring. It does increase CA events in some users that are sensitive. If this case is happening to you treatment becomes a balance between CA and obstructive events listed above.

What happens is the CPAP and EPR has increased the efficiency of your breathing, not a bad thing but it increases the clearing of CO2 out of the blood lowering the CO2 level to below the apneic threshold. It is the CO2 level that provides our drive to breathe. We will see by what happens when you try EPR = 1.

Last nights data.  Overall feel decent from sleep.  I woke up around 11 pm with mouth open and dry so I taped my mouth.  AHI is back down a bit but CA still over half of total events.  EPR changed to 1. Thinking these are emergent CA events that will dissipate with time, or something we should keep experimenting with changes to improve?

Looking like your CA could definitely be Treatment-Emergent but lets look at a close up of a couple of CA events, a 2-3 minute view please, just click on an event until you get that close.

Not a significant rise in obstructive events to go with the reduction in Centrals.

Try this for a total of 3 nights (1 night does not make a trend)
Then try EPR=0, post first night then try for a total of 3.
Then you pick which felt best for you.
