Help with AirCurve 10 VAuto Pressure Settings - timkay - 05-11-2021

Hello everybody!

I've been using the ResMed AirCurve 10 VAuto with a ResMed AirFit N30i mask at night.
Throughout the day, my stomach feels bloated and I fart / burp a lot.
I've been told this could be because the pressure settings on my CPAP machine are too high.

What are the recommended changes I should make to my CPAP machine config?

They could be too high. Your pressure is alot lower then mine. I get a little gas when I wake up but nothing to bother me I'm 14 IPAP over 11 EPAP. Your dreamwear has a nasal pillow that might be the issue, driving air directly into your nose. Are you a nose breather? If so, then I would say you nead over the bridge of the nose full face mask would be better to alleviate too much air through the passages to your stomach. Also is that the mask you had your study with? Wondering if you mentioned it to the tech after your study. Changes to your pressures, not at this point, I would get rid of the dreamwear mask. But maybe try down to 10 IPAP.

timkay, your Vauto is set to S-mode with EPAP 7, IPAP 10, so you have PS 3. Your events are very low and there is no suggestion of snores or flow limits, so we can lower your pressure, and probably improve your CA events. We can put you into Vauto mode at lower pressure and we will se what the flow limits really are (they don't record in S mode. Lets set mode to Vauto, drop EPAP min to 5.0, IPAP max to 8.0, PS 2.0, and use trigger sensitivity on High. Try it one night, post results tomorrow.

Vauto is just as bad as APAP. Set pressures are best.

Not always, in fact most cases are the opposite. Running in VAuto should give better results in most users because needs vary with different sleep positions, sleep stages, etc.

Just adjusted the settings and will post the daily report tomorrow.
Another question I have is about the leak rate. I changed from a full-face mask to a nasal pillow near the end of February.

Looking at the OSCAR data, I noticed that the leaks started increasing a lot after the change.
However, the Large Leaks are still rare.

Should I be worried about the leaks? Would changing back to the full-face mask help the my high pressure?

Your leak rates are higher some of the time, but not always. A full face mask may or may not help. Since there's more surface area, it's going to be more prone to leak, at least in most uses. If the leaks tend to be high frequently, then yes definitely address it somehow. Even moreso if the leaks are disturbing your sleep.

A mask change can be as simple as a redo in the adjustment, a new cushion, or a different size. If those don't address the larger leaks, then maybe another model should be looked at. But to me your leaks are a problem sometimes not always.

Remember, leaks that are higher can hinder accuracy and cause disruptions. My rule of thumb for this is if the leak graph is near or above 24.

Are you using the tamp?  It looks like it for about 20 minutes?  If so you stoped and it started again which gave you 40 minutes without getting therapy. If you are using the ramp try to stop if it cut the time on it as much as you can tolerate. 

The charts look good, but the main thing now is how you feel. Which settings are giving you the best results?

Okay, we improved your AHI and eliminated the CA events. Did this feel better and help with the aerophagia?  We are doing these changes to improve comfort.
