Blowing air from mouth - mtyakker - 05-28-2013

Looking for help. Had my Res Med S9 Autoset for about a year now. When I first started out with it it was great. Have not been back to my sleep specialist so the machine has not been touched since recieving it from Nationwide Medical. About two months ago I was being woke up at various times of the night by blowing air coming out of my mouth. Using the nasal pillows. I recently started using the chin strap, but it is uncomfortable to wear along with the head gear nasal pillow. My question I am asking myself is, is the preasure too high with this machine, do I need to go to a full face mask, can I adjust the preasure myself. Any input would be appreciated.

Have you changed your pillows fairly recently? I notice that in your profile you have unsure for your pressure; if you knew your pressure, that might help others to help you. Hang in there for suggestions.

No, it's not the pressure. If you have left it where it is, then that's not it.

What has happened is you have just started sleeping with your mouth open. It happens to all of us at some point. Some more often.

Use the chin strap until you think you have stopped the habit then try sleeping without it. You can look at the data from your Autoset (as long as it is an Autoset and not Escape Auto) and see how the leak rate is.

There's some others who have beaten this habit by training themselves to keep their tongue forward, against the front teeth. I've not tried it so I can't explain it. Hopefully they'll see this and come to help.

Here are some options. Try adjusting the chin strap. Maybe it's too tight. Try wearing the chin strap under or over the mask to see if one is more comfortable than the other. Try a different chin strap, there are several different versions available.

If the chin strap is so uncomfortable that you cannot wear it, then the only option available is a full face or hybrid mask.

The bit about training yourself by tucking the tip of your tongue against the roof of your mouth behind the front teeth may work. I found it useful as a newbie, but I find I can go weeks without needing a chin strap and then find that I can't go more than a few hours without it. Now that I've adapted to it, I just wear it all the time.

One other thing prior to going to a Full Face Mask is that you may want to check what your humidifier is set to. Sometimes you can get to a point where you aren't getting enough humidity (especially with Nasal Pillows) Then you can get rhinitis starting and once it starts, you may need to open your mouth to breath comfortably. What is the humidifier settin? The one question from above is have you changed the size of the nasal pillow lately? If you have moved to a smaller pillow, you may be getting some air hunger because of a decreased amount airflow through a small opening. Sometimes a Full Face mask can result in requiring a higher PAP pressure, so it is important to monitor your AHI if you decide to get a FFM.

I agree Shastzi. I had the same problem, that's why I have a full face mask. I'm running at high pressure and had a nasal and a Fisher Paykal mouth only device that wouldn't work either as I would literally blow the nose clip off in a few minutes.

Full face mask allows you to breathe through your nose or mouth
Use whatever feel comfortable for you and seal well without over tighten the straps
Sleep-well

I had a similar experience of air coming out of my mouth with nasal pillows. I haven't tried a chin strap; I am using a RES MED QUATTRO full face mask and it seem to leak under my eyes and next to my cheeks unless I pull the straps very tight. I would like to be able to reshape the plastic frame to fit my face better -- has anybody tried this?

I started out with the Quattro FFM and the sleep tech explained to me that in order to get a good seal, you must have the machine running at full pressure because the edges are two layers of silicone that need to "inflate" (for lack of a better word)with air to create a good seal.

If that doesn't help, on the Mirage Quattro website there is a template you can print out and cut with scissors that indicates what size mask you need. If the mask size is wrong you won't get a good seal because of your facial contours.

Another variable is how dry your face is, if your face is oily or you wear any kind of moisturizer on your face to bed the mask may not seal tight enough.
