Has my machine just plain stopped working? - mgcunn27 - 09-23-2021

I've been trying to figure out why I'm feeling so fatigued lately and have been waking up during the middle of the night after using my resmed s9 successfully for 6 1/2 years. I've never even tracked the software data on it until this month but because I've been feeling so bad I decided to check and investigate what it could be.  At 5:20 this morning I woke up not suffocating, but with a feeling of breathing really fast and feeling short of breath(Almost like I had to try hard to breathe in deep). Which after standing up and using the restroom, the feeling went away after 10 minutes. What is concerning me is when I checked OSCAR to see what happened before I woke up, my respiration rate seems to have tanked all the way down to 2.8(can see in photo below). The pressure on my machine at this exact time labels very little to no response to my respiration dropping. (It moved up like half a point) Also my expiration rate is labeled at 10 while my inspiration is at 1.22. These numbers look very odd to me. Now I'm wondering if my machine just isn't responding to the required pressure I'm needing when I start to enter the phases of REM sleep. Are these stats something I should be concerned with? I'm pretty nervous now about sleeping tonight.

I wouldn't hit the panic button jut yet. With your S9 being at 6-1/2 years old, it may be coming to its end-of-life. How many hours are on it?

A Resmed responds to flow limitations with added pressure. Your flow limitation is very low, thus could be the reason of it not increasing the pressure. Your chart looks like you may be either chin tucking or back sleeping. Are you using anything to prevent either condition? Also, do you have a recording oximeter? This should be in the "things-to-consider" list.

Are you on any meds that might affect you?

I'm sorry that all I seem to be providing are more questions and very few answers.

It's got 18,000+ hours of use. I've used it almost every single night I've had it since May 14th of 2015. Maybe missed a total of like 10 nights in all that time.

I have been back sleeping some lately though not intentionally. I wake up on my back even though I go to sleep on my side. The only oximeter recording I've been using is my apple watch which hasn't charted my oxygen going below 92 in the night(Though this is a periodic check and not a constant reading). Also, I caught Covid and went through a mild case of it last month, however the anxiety over the whole thing caused me some bad insomnia, which I was prescribed 50 mg of trazodone to help with. Since then I haven't had an issue falling asleep. Questions are all fine I don't mind answering.

Another thing I've noticed is that when I put the mask on while awake and hold my breath, I don't feel any pressure kickback and I'm not sure if I'm supposed to.

Also what exactly is flow limitation? That's one stat I'm still confused on.

18,000 hours is high, but several S9 owners have reported going to 22,000 hours.  They are robust little devils.  Since you mention having Covid, have you had a pulmonary function test since your recovery?

If you can try to stay off your back (easy for me to say!) and try to sleep on your side, left preferably, should aid in reducing the OAs.  When we are on our backs and relaxed during sleep, gravity is not our friend.  This is why the airway will collapse. Additionally, the pillow can cause your head to extend forward, causing a chin tucking condition, that also restricts air flow.   If back sleeping continues, you may wish to consider a soft cervical collar (SCC). Even thought you are still on your back, the SCC will at least restrict the chin tucking. I have provided a link below on this subject.

Instead of me trying to provide an answer on Flow Limitations, here are some much better explanations on the subject.  (If it tried to explain it, you might windup thinking that I was telling you who shot J.R.)

Also, here is a chart of different flow limitations:

Time for a new PAP. 18k hours is at or near end of life.

I haven't had a pulmonary test although I was lucky and didn't have any shortness of breath or breathing issues at all throughout the entire illness. Thx for the info, I will purchase and try out a cervical collar as well as focus on sleeping on my back. Throughout this entire time in my naivety I thought as long as my machine was on it didn't matter the sleeping position I was in.

Yes, the ResMed AirCurve 10 VAuto is what you're looking for. It should be easy enough to do setup. If your current settings are OK, it's more or less copy and paste ish.
