Hello, Help with data [VPAP ST-A] - Aishor - 12-22-2021

Hello everyone, I am Spanish unfortunately I do not speak English if you allow me I will try to use my google friend to translate

I have been improving my health for years, first aquifers, then hypertension, with obesity, now for years tachycardia and more

I recently visited the public insurance cardiologist, who sent me to the sleep specialist for evaluation, this means more than 8 months waiting for the next visit to the doctor

I have not been able to wait that long this badly, so they lent me a resmed S9 machine with a resmed nasal mask which I have used to see if I have problems with apnea, which with these data you can already see something.

Can you help me, as I see this data I am wrong but not so bad

Male 46 Years, 1,69cm hig & 103kg

Welcome to the Apnea Board,

A few things I'm seeing that will be a help. Leaks looks like a little problem. Try to adjust the straps so they're not very tight, but just so there's some leak control. We don't want to create pain, and a too tight mask will do that and make it leak more.

However, I think the bigger issue is the clusters of events, very likely a Positional Apnea issue. If you're using a thick pillow, consider a thinner one. Or there's a soft cervical collar that will prevent your neck from kinking off the airway. I have a wiki link to this below in my signature.

Another thing, you may benefit by bringing up EPAP a little, maybe trying 5 to see if this helps reduce the events.

Aishor, you are on an advanced positive pressure machine that is designed to maintain your breathing volume. This is not CPAP, and it would be very helpful to know more about why you need this kind of ventilation support. We can see on the flow chart (tasa de flujo) that you alternately hyperventilate and hypoventilate on a regular period at about 2.5 times per minute or once every 24 seconds. Your respiration rate is relatively high at 19 breaths per minute. It's my opinion that this machine may be set wrong for you. The exhale pressure is set to 4-cm and the minimum pressure support is 6.0 cm resulting in a minimum IPAP pressure of 10.0. We can see the maximum IPAP is frequently 22, resulting in machine pressure of 22/4.0 (IPAP/EPAP). This would be extremely stressful and abnormal

Your machine appears to have the breath rate set too high and the alveolar volume target is resulting in a minute vent of 8.5 L/min. Are you certain that your doctors intended you to be put on a ventilator? If the intention was CPAP or BiPAP you are using the wrong machine. What conditions are being treated? Are you aware this machine is breathing for you? If you are able to breathe spontaneously, we should change to some different settings.

After the visit with the cardiology specialist, to treat my tachycardias and arrhythmias, consulting an echocardiogram and my electrocardiograms, he suggested apnea, and sent me to the specialist.

I have consulted the waiting with the specialist in public medicine, and I have more than 8 months of waiting, to be able to see the specialist, and then for the tests to be sent to me, there will also be a waiting list.

That is why I am not willing to wait any longer, there are days when I cannot go to work losing money, I do not have enough, I live from day to day, and my health is progressively suffering because of that, I have taken measures, I have borrowed a unit from relatives to start to treat me.

The first thing I have is a great improvement in health, my heart is slower, and my vital energy has risen, fatigue and sleep have decreased

After using the machine and seeing the statistics, it is clear to me that I have apnea, but that is why I come to this forum, to ask and guide me since in Spanish I have not found more than faccebook groups or telegram.

First use the autoconfiguration function that the machine brings, plus the detection of the patient's values

Then adjust values to feel comfortable with the breaths in addition to allowing self-management thanks to the automatic system of the machine

little by little I am trying to document myself and improve

Thank you.

If it helps, tidal volume and backup breath rate can be manually set. I'm guessing your ST-A did what my test did. The Learn mode chose too high values and will need to be lowered manually.

What values are in backup rate and tidal volume?
