AHI increasing with no change to settings - Sleepygrl - 12-31-2021

Hi everyone! My AHI went from 2.0 to 2.8 to 3.0 to 3.5 to 3.9/hr in the past 5 nights respectively. I haven't changed my settings. I know there will be fluctuations but I'm wondering if there is an explanation for the slow increase. My pressure is set at 5-18, ramp/EPR off, Climate-auto, tube temp 76°. TIA for your help!

A chart would tell us what component of your AHI has increased, and might offer some clue as to why. Without that, it's just a guess.

It looks like events are being a bit variable. CA tend to do that, but in your case, also Obstructive, Hypopnea, RERA, leak stats and large leak are moving up and down some.

I don't know if this will help the variableness, but maybe it can boost comfort. It doesn't appear EPR is on. Consider EPR 2 full time with a slight increase in Min pressure to 6 to allow EPR room to do its work.

I definitely think you are going to like to use EPR with a 95% flow limitation of about 0.13, you're experiencing flow limits and hypopnea through the night. You have been increasing pressure, and that is not helping, but EPR will. Set your minimum pressure back to 8.0, maximum pressure 16, turn on EPR Full Time (not ramp only) at a setting of 2 or 3.

If you're using more than one pillow, or if the pillow is tall and firm, it may be causing you to tuck your chin. The curve in your airway will restrict air-flow and cause the clusters of obstruction we see on the 27th and 30th.

You are experiencing a positional element (clustering) to your numbers. Pay attention to your pillow situation as Sleeprider said above.

I took your advice...I was not only going to increase the pressure and turn the EPR back on as you recommended, after reviewing my initial paperwork that came with my machine, in tiny print at the bottom of the invoice I found that my doc had my initial pressure set at 12-18, which I had not noticed and changed multiple times in trying to teach myself how to use this machine. I set it back to this pressure last night and had 1.7 events per hour!  The lowest yet!  I'll keep the pressure here for a few days and see how it goes. Thanks for your advice!
