Sharp inhale 15min after going to sleep? - McPhearson99 - 07-08-2022

Background: 27y/o male, diagnosed with upper airway resistance after a in-clinic sleep study due to an RDI of 11/hr and an AHI of 3.4/hr (AHI during REM sleep was 11.8). I'm using a secondhand VPAP III in CPAP mode until I can get my hands on a modern AirSense. Currently I'm sleeping every night with a nasal pillow and the CPAP set to 11inH2O.

The specific problem I need help with is that frequently when I'm going to sleep, 15-17min after I close my eyes I wake up almost fully with a sharp inhale through my nose or a light snore. As far as I can tell, I'm not snoring prior to the inhale/snore that wakes me up. I usually fall asleep fully after 1-3 of these events. Does anyone have any idea of what's going on and/or how to fix this issue?

If your problem is UARS, then you would benefit from the use of pressure support in VPAP mode rather than CPAP. A spontaneous strong inhale is a normal response to an increase in CO2, so you may be experiencing shallow breathing (hypopnea) or apnea. I think you might consider changing to VPAP mode with EPAP 8.0, IPAP 12.0 and see what that does for you. A newer machine with data would be a big help vs your museum piece.
