The form associated with sleep apena testing - dlnewhouse - 08-31-2022
I just received a CPAP machine and a sleep testing mask at the same time. I wasn't sure what I was looking at. One of the problems is that the "registration?" form for the testing was lost? I ordered the equipment through my doctor. Does anyone know exactly what the form is that you use to register your 2 nights of testing?

You might be better off calling your doctors office or DME to find out what form they are talking about. It may be insurance related.
