Been on therapy for 8days no luck! - badge - 04-13-2015

Hi I've been on therapy on 8 days now.. i've been waking up at night to literraly either turn off the machine or change the pressure as i've been gasping of air.

as well there are times where I wake up when the elbows on my Airfit f10 mask isn't connected anymore.. i'm not sure if i did this on purpose.

My pressure is set at 4cm with PS 4 Min EPAP 15cm IPAP 4cm. most of the time on sleephead data rate usually at 8cm - 12cm.

untreated AHI was on 31, and now it went down to 16 - 15, I only had once where it went to 1.6 AHI.

Pls. let me know and would be happy to hear your advice.

*on a side note is it usually normal to have you s9 machine turn on automatically once it's plugged in.

The sentence in bold does not make sense to me.
Perhaps some commas would make it more clear.
There should be:
IPAPmax
EPAPmin
PS
and if using ramp, the starting pressure.

If you're using ramp, turn it off. Starting the ramp at 4, most everyone finds they cannot get enough air.

It's not normal for the machine to just turn on when plugged in.
AutoStart may be enabled. If so, it's not supposed to start until you draw a breathe through the mask. I suggest you turn autostart off and just use the start/stop button. If it continues to start at power on, with autostart off, then it has a problem.

WELCOME! to the forum.!
The reason your machine turns on when you breath into the mask or off, when you take your mask off automatically is because you have those features enabled, they can be disabled.
Hang in there for more responses to your post and much success to you with your CPAP therapy.

Hi here are my settings:
IPAPmax - 15
EPAPmin - 4
PS - 4
and if using ramp, the starting pressure is 4.

How can i disabled the auto start?
Btw my physician me for prescribed me for CPAP Auto; but been sold by DME to go for VPAP.

Now i'm just wondering i'm only 29. Should I return the VPAP and get a CPAP Auto instead as I feel i'm missing out on something.

My lungs are normal no CA only OSA. I used to smoke but quit 5years ago.
BMI's normal as well.

Any recommendations? I've heard Sleepcube/Intellipap is recommended brand as well. Pls. let me know your thoughts.

The VPAP Auto can act like an Autoset (Resmed's Auto CPAP.)
It's capable of running a wider spit between IPAP and EPAP.
So, you're not really missing out on anything.
BTW -- your S9 VPAP Auto is set to Auto mode?
It can be set to run in straight CPAP, or Bilevel mode S or Auto.

The the issue I see is your EPAPmin of 4. with PS =4, that means you IPAP starts at 8 (EPAPmin+PS.) Bumping up the EPAPmin to 6 will start the IPAP at 10 cm-H2O.

Autostart is in the clinician's menu. I suggest you request the Clinician's manual by email. Look near the top of the page for "CPAP Setup Manuals."

Hi there thanks for the response,
I've been visited that option as well and do the math. however the thing that is bothering me is about the Ti Min or Ti Max (timed) as it control both ways although I do understand the EPR relief which helps. but i always encounter gasping for air at night and sometime need to turn it off so i can catch my breath. not without the Leak but because of the Timed Breathing.

If I can disable this i'd be great. as i can only access the CPAP mode.. (without Auto) and set between 4-25.

As recommended previously, you need to increase the min EPAP until you no longer feel air-starved. 6.0 might be enough. I currently use 7.5 or 8.0. You really shouldn't need to adjust ti min ti max settings. You can read about how those settings change the rise time and ventilation rate on page 23 of this document. http://www.resmed.com/us/dam/documents/products/titration/s9-vpap-tx/user-guide/1013904r3_s9-vpap-tx-sleep-lab-titration_user-guide_glo_eng.pdf

If you are having problems with this aspect of using your bilevel, I would rather have you call your sleep specialist or DME than take advise off a forum, but we can try to answer your questions.

Timin and timax are almost always best left at default. Timin is the minimum time the machine will stay at IPAP; and Timax is the max time it will stay in IPAP. Most people's breahting patterns fall well within Timin and max -- so, they are not a factor.

The trigger sensitivities and rise time are almost always best left at default.

Badge: You may find the VPAP more comfortable with easybreathe turned on. The transitions from IPAP to/from EPAP are more gentle and natural with easybreathe turned on. When it's on -- rise time is grayed out.

Consider using software and posting some data graphs. Let's see where your pressure is when events are flagged.
