Number five may be alive - but the site says down for maintenance since 5-3-2015. Nobody's answering the phone either - it just goes through to voicemail. Does anybody know if they are permanently off line?

I would just put the Supplier #5 on "RESERVED" and fill it in with someone who is in business. I figure two months is quite a while to be off-the-air for an online DME. They can always be added back later.
