RE: 47xa Therapy Thread: Rem-Related Sleep Apnea
47xa, you have relatively good results, but we can make them much better with some minor modifications. First, I can see the flow-chart seems decapitated on the top (inspiration) peaks. This is the result of flow limitation, and we can address that by using the exhale pressure relief (EPR) feature of your Resmed Airsense 10 Autoset. This will make your CPAP more like a bilevel with inspiratory (IPAP) and expiratory (EPAP) pressures. The difference between IPAP and EPAP is called pressure support, and it helps to improve inspiratory flow limitation by assisting the breathing effort with more pressure as you inhale. As an additional benefit, it will make therapy more comfortable. To activate it, enter the clinical menu and turn EPR on, Full-Time at Setting 3. This will change your pressure from 10.6 to 14.0 into bilevel pressure 10.6/7.6 to 14.0/11.0. Let's take a look at what that does for you before making other changes.
Your apnea occur in brief clusters, especially around 21:50 in your chart. Clusters of apnea are probably not REM related, but positional. Read our positional apnea article here, and the article also discusses flow limitation. https://www.apneaboard.com/wiki/index.ph...onal_Apnea
