Aircurve 11 ASV cant tell I'm breathing.
#1
Question
Aircurve 11 ASV cant tell I'm breathing.
Hello,
I awaken multiple times, and the machine very forcefully attempts to get me to breathe in it's established pattern, making it impossible for me to fall back asleep. I have to power down, and restart to enter ramp-up, hopefully to fall asleep on the first attempt.
This machine obviously cannot tell that I am breathing.
Any thoughts appreciated.
Thanks.

Welcome to Apnea Board,

I would suggest you get the free OSCAR program so you can see the sleep data. You'll also need to attach it to a post. We'll need this data to understand what the ASV is doing, what it's trying to accomplish. There's not enough info in this post.

To get the data to OSCAR, you'll need an SD card inserted in the slot on the left side of the ASV before sleeping to capture detailed data.

I've used the ResMed AirCurve 10 ASV and it'll be much the same with settings. You'll need to know what it's set to. You need to tell us what X is for each of these settings:

Mode X
EPAP pressure X
PS X

If you're using Ramp, it'll likely hinder ASV therapy.

Attach OSCAR, etc.

The Resmed ASV machines are pretty good at setting rate and volume targets, but you're describing problems while you're awake. A common problem results from masking up before you have settled and breathing normally. Deep slow breathing gives the Resmed ASV a false target, and this can be very confusing. Resmed uses your last 90 seconds of respiration rate and volume to set its targets for respiration, so if you're not relaxed and breathing normally, it can seem the machine is at odds with your respiration as sleep onsets. Hard to tell you what to do, but this is an anxiety issue, not a hardware issue.

Thankyou for your kind reply to an un prepared newbie! Frustration motivated my shabby post.
I have turned off ramping. I will post my settings, and get a PC and get familiar with Oscar.

Stabimka, something I frequently advise to new users of ASV is to not mask up until your breathing has settled down from preparing for bed. The process of going to bed often involves dressing, getting into bed, moving around to prepare the mask and covers, and if you just pop the mask on, you are often breathing hard from that effort. It's important with ASV to be patient and settle, maybe read a few minutes to allow normal, relaxed breathing to resume. The ASV uses the last 90 seconds of your breathing rate and volume to establish its targets. If you are still breathing hard, that is the target it sets, and that is what you described. That can upset much of the night. So take some time to relax and many of the problems will go away and you won't feel like you're fighting the machine. Anytime you feel at odds with the timing or pressures, take a few purposeful breaths to retake control or stop the device, relax and start over. You control the ASV, but sometimes it can feel like it turns the tables on you.

Agreed with this. I myself needed to pause a minute before mask on when on ASV. With no manual time settings, the ResMed ASV only follows our pace input.

This is very interesting.  You indicate that you awaken multiple times.  You seem to imply that you are needlessly being awakened, perhaps because the AirCurve11 is ramping up the pressure to get your to breath.  Well, if you were asleep, what makes you think your were breathing?  The most likely scenario was that you stopped breathing while you were sleeping, which of course, is a very common phenomenon for someone who has Sleep Apnea.

Now, for the other problem, you seem to be indicating that the pressure is so high you are unable to go back to sleep.  I can definitely relate to that.  That is a problem I had from the very beginning of using a CPAP machine more than 13 years ago.  I simply was unable to sleep with the machine due to the high pressure requirements.  Eventually, I had to take sleep med to combat this problem.  Not an ideal permanent solution, but better than no CPAP and no breathing.

I suggest discussing this option with your doctor.  Perhaps after a period of time with the sleep med, you may get used to the CPAP and will no longer need the med.

I suspect that I was breathing before awakening, because I have had a few nights with "0.0" Ahi , and have , on those nights, as usual, awakened, restarted the machine after it had been very forcefull and out of sync while I was attempting to fall back asleep.
By the way, I slept in my recliner (legs level) and Ahi went to 0.0, now bouncing in tenths !!
