To build on Jay's comment, you are using a pressure of 4.0 to 7.0 withe EPR 3 and a ramp time of 15 minutes. That means EPAP can never rise above 4.0 and that is the source of the obstructive apnea. We need higher EPAP to stent the airway and avoid the OA. A rule of thumb is that the minimum pressure on an Autoset should be at least 4.0 cm plus the EPR setting. For you, that is a minimum pressure of 7.0 which allows a 7.0/4.0 (inhale/exhale) pressure. With EPR 2, it is 6.0/4.0. I recommend a minimum pressure of 7.0, maximum pressure of 10.0, EPR full-time at setting 3 and Ramp on Auto or off.
