AHI down to 1 but still getting wakeful CAs
I have managed to get my AHI down to 1 but treatment emergent central apneas are still happening and they wake me. Im not sure if they wake me every single time but it happens a lot and it disturbs my sleep.
I am running the machine at a single pressure with EPR at 1. I did try having EPR off once with different settings but it made it difficult to breath and I felt like I was struggling and I had a very difficult time falling asleep. Would VCOM help in this situation?
I'm using a F&P Nova Micro with mouth tape.
I was looking at my flow rate and a most waves are non sinusoidal.
RE: AHI down to 1 but still getting wakeful CAs
I myself don't see the CA as being a big issue. The flow limits however look like they might be a bit of an issue. To address those needs more EPR.
You have had the CPAP how long? What were the details regarding the sleep study? You do have you copy of that detailed report right?
Mask Primer
Positional Apnea
Attach OSCAR, etc.

I have had the CPAP 3 weeks now.
I have a physical copy of the report but not the raw data. It did not show any central apneas. Overall ahi was 11 and 25 during rem. Doctor prescribed a pressure of 10-17

RE: AHI down to 1 but still getting wakeful CAs
OK, understood.
It's not the raw data that is necessary. It would be a detailed sleep study report, including the event table with the type and count. Then the doctor's recommendation after that. This is in regards to the Central Apnea.
It is probable your small amount of CA shown in that OSCAR is the treatment emergent Central Apnea variety.

RE: AHI down to 1 but still getting wakeful CAs
You had Zero Obstructive Apnea, Zero Central Apnea, 82 Hypopnea from Total Sleep Time 458.5 minutes. 335 total arousals.
Any Central Apnea you see in OSCAR would be what I'd call treatment emergent Central Apnea. The CPAP is causing this little bit of CA due to the slight pressure increase that's flushing out a bit of CO2. Your brain compensates by adding breath pauses until the CO2 drops enough to trigger a breath. This equals a CA on the OSCAR chart.
Regardless, the CA component was only cAHI 0.92, very low and I'd not give much allowance to have this dictate your therapy.
Your OSCAR indicates with the Flow Limits you'll need to add more EPR, you only have EPR 1 now. Try 2 then 3. You might need later a slight pressure increase and/or adding a pressure range, especially if comfort diminishes.
