Request OSCAR Analysis
Long time lurker, first time poster. Have been using cpap on and off the past year, really trying to give it a chance. Finally have a good configuration so that I've been able to wear it without mostly being woken up in the middle of the night multiple times. So as of today, I'm roughly approaching the 3 mo. mark of consistently using it. Am able to keep the thing on throughout the night, but still wake up feeling crappy. Finally went through a ton of my notes today from the past... and just feeling extremely bleak. My untreated AHI is 7 through an at home study (Was just mailed an oximeter to go around my finger, no RDI measured) but the severity has always felt much much worse than the seemingly 'mild' diagnosis.

What I've tried, and how it has or hasn't worked:

* Current Setup


My current setup is an Aircurve 10 with a hybrid face mask and a tall cervical collar to keep my mouth shut and stop my jaw from opening. Don't know if that pressure is considered high, but it feels very comfortable along with the pressure support setting. 

* Past Setups & Main issues

My biggest issue and why I use the FFM and cervical collar regardless of my good nasal breathing is my tongue. Trying the nasal mask previously, with extremely strict tongue posture and training throughout the day, the second my jaw slacks slightly, my tongue falls back and blocks my airway, or gets thrashed about by the air and blows up my mouth. I've tried tape, chinstraps, cervical collars, over many nights and tracking different degrees... but I've accepted that a FFM seems to be the only thing that solves this for me. 

To solve this, I've used chin straps before to varying degrees of success. The best one I'd used was a red ruby style one that stretched, but it was getting frustrating as it would stretch out after a week or two since I needed it fairly tight.... and was not a great experience to suddenly discover the great fit last night was slightly too stretched out right as I was heading off to sleep Sad

My main solution to this is a FFM (AirFit F30i) and a very tall cervical collar (~4'') that my chin rests on with a decent bit of pressure keeping it closed. This mostly works... but after taking notes every few days and really looking back, it's hard to tell how much CPAP is working which is extremely disheartening. The main symptom I've noticed improving a bit is waking up without a headache, but still feeling very fatigued after work on days where I notice my sleep is slightly off.... which doctors don't seem to be of too much help even though I have good blood results and run / exercise every other day.

Is there something about these charts that stand out? Would love to get feedback, I'd tweaked PS based on flow rate discussions I'd seen here before and got the collar idea which has been great... hoping it's small things that I've overlooked. Attached are two nights that have looked good on charts (202502*) but then later in the day I've crashed... so fairly common chart. And the last (202501*) is a day I have from my notes as feeling great, hard to find others as I haven't had one in a while.
