RE: a Thank You and my first OSCAR post
well hi all,
I was going to give this a couple of days but couldnt resist looking at the data.  All I changed was bumping the lower limit up from 6 to 7.  This had made things worse just looking at the numbers however is the positional issues?  I had not had that before but since it is CA I didn't think so
Any input or guidance would be great
Thanks,
Dave

If your OA’s are controlled and you have all those CA’s. Try a lower EPR.

As i understand it. Positional apnea’s are OA’s not CA’s. CA’s are ‘clear airway’ so no blockage.

RE: a Thank You and my first OSCAR post
Hi all
enclosed is last nights chart.  I can zoom in any areas if that will help.  This large leak rates is when I rolled on my back and breathed thru my mouth until it woke me.  I have been working to sleep on my side as that was what my sleep study and my wife has indicated that before the CPAP I did better on my left side.  This hadn't happened since I started the CPAP.  My settings are 7 min pressure and EPR of 1, I was afraid if I turned that off I might suffer on the exhale since I am still new at this.
Any thoughts on the CA events or should I give it a couple of days to see if it changes?
Thanks,
Dave

As you’re still having some OA’s & H’s and as your median pressure is 8.5. Try raising the min to 8 and see if that helps.

You might try staying at EPR 1 as more tends to increase CA’s and you have quite a few more than in your study. So they are likely treatment CA’s. I went through the same as you. Apnea’s did well under apap, but my CA’s went from zero (in study) to well above zero. Drove me crazy for a while.

Thanks Super
I will give the higher pressure a try.  I have read mixed reviews on what to do with the EPR when it comes to CAs.  When I first turn this on for the night I seem to struggle with the exhale part.  After a short bit it seems to all work good, it is probably just me.

RE: a Thank You and my first OSCAR post
ok, I am back with more info.  2 nights ago I bumped the pressure to 8 and the EPR to 1.  I didn't include this graph but can if it helps but my AHI went down to 4 so it was a vast improvement but I was uncomfortable.  It felt like I needed to force the air out so I ended up taking the mask off a couple hours early and slept the rest of the night.
Last night I went the other extreme and added EPR of 3.  I was so much more comfortable and made it much better however it did cause my EPR to jump.
I am using the nasal pillows and it is comfortable and I feel my leaks are minimal.   I have a different mask that covers my nose and mouth but was avoiding it I have a beard adn I don't think it will be as comfortable and seal as well.
I will also add I wasn't aware I had these issues over the night however I could tell I had not slept as well when I was getting up this morning. I assume a lot of the "junk" at teh end is when I was waking up. Sometimes I will just leave it on and usually sleep another hour but I pulled it off once I tossed some.
Let me know your thoughts.
Thank you,
Dave

RE: Custom 68 Therapy Help Thread
Hi all-
2 nights ago I tried a different mask.  I wanted to try the full mask as sometimes it seems my nose is a bit congested giving me concerns.  That was a total bust on that round.  I went back to my nasal pillows and got a good nights rest last night.
I wanted to switch things up and limit my pressure changes to see if that helped the CA numbers.  7 left me wanting more air early on so I went to 7.4 with a max of 9 and an EPR level of 2.  I had chased pressures up previously but it wasn't comfortable for me.  This netted me a good sleep but still want to lower these values.
Tonight I will bump both lower and upper some again but will go slow.  I am 3 weeks into treatment so I realize things may even out as I get further into the treatment.
Thanks for following along and any suggestions are certainly welcome and wanted.
Dave

RE: Custom 68 Therapy Help Thread
Here is the last night version.  This is the first time I had these leaks.  I may have been a bit more restless.  it seems pretty common for me to have several CA events as I am falling asleep.  Any thoughts on a direction or stay the course?
thanks,
Dave

RE: Custom 68 Therapy Help Thread
Looking back it appears I have done better with a range of 8-16 and EPR of 2.  It appears my flow limits are pushing my pressures up in this last iteration.  I am not sure why those have been changing.
I know I have become more comfortable so I am moving around more, this included to my back causing some mouth breathing.  If I lay on my left side/stomach I can control any leaks otherwise I get some open mouth causing these large leaks then waking me up.  I have tried the chin strap that seemed to help at the first then as the night goes on it seems to loose its effectiveness.  I will try to adjust that.
Do these graphs tell you anything that I can improve on?  On the zoomed area, I don't see anything that should cause all the CAs.  That is what continues to cause AHI higher numbers.

Thanks,
Dave
