AHI is the easy part with ASV.
Auntimame, with the small residual hypopnea, I think a EPAP min of 5.0 and PS min 2.0 might fell and look better. Your median EPAP is nearly 6.0 and I think a little elevation in that setting will help, and the PS min is mainly for comfort. The settings are not shown. What is your PS max?

PS is 20 with IPAP of 4 and EPAP of 35

Okay, your maximum PS is significantly lower than that. Increasing EPAP min by 1.0 is based on titration guidelines. A little more PS min is intended for comfort and to reduce some of the background flow limitation.

Thanks for the info and the encouragement. I did try the Resmed under the nose full face but it was really bad with leaks even with gluing or taping it to my face! I am keeping it just in case of sinus or cold problems I think it will work okay when I am sick (or at least I will be able to breathe through my mouth). I will look at the other masks and see what they look like. Have you heard of the SleepWeaver mask? The Respiratory Therapist told me that might be a good one to try. I do try to use a mask for a month just to see if its me or if the mask just doesn't work for me. Also my Respiratory Therapist told me I might try a pediatric X-large mask because I have a small frame. just wish this was a simple as my doctor tries to make me believe it is!!!

Congrats on a really good night!
I will be trying the ResMed F30 FF mask next month. I tried the SleepWeaver some time ago. Leaked like crazy. I will report on my experiences with the F30.

Mask leak is very important with ASV, you can see on your chart how the leaks follow the pressure rises. The machine has a mask fit function that blows high pressure that will help.

For auto-cpap, from machine data or software. You can set the min pressure 1 or 2cm below 95%. Or clinicians commonly use the maximum or 95% pressure for fixed pressure CPAP, this can also be used for min pressure. https://aasm.org/resources/practiceparam...rating.pdf

Raise your Min PS. Default setting is 3.0. Right now you have it at 0 and are spending a lot of the night there. High CO2 during the night could explain your lethargy and headaches, and if this is the cause, a higher min PS will help.

well I have stuck it out and done a few things that I thought I would not even try but they worked! I have had 3 nights of 0 AHI! I am hoping these are true readings and not something wrong with my machine! I have tightened my chin strap up (that does take a little work, too tight you wake up in pain and too loose and it is off your head and roaming around the tubing!) and have been taping my mouth since I have been taping my mouth the AHI has been at zero.
I didn't adjust the settings on my machine because I am currently renting this machine from Aerocare (they won't allow purchase of a machine for 6 months BLAH). If I owned my machine I would absolutely adjust the settings. I am going contact my doctor if my AHI continues to be zero and see if he will adjust the settings or what the next step is. Can you go off the machine if your AHI is continually zero or is it the machine that keeps them at zero? Still reading and researching all I can about sleep apneas, especially when a person has multiple or all forms of apnea. Thanks for all of the help and encouragement. Mamie |
