Getting Started with a Resmed Airsense 10 Autoset

First post here!

After getting more and more tired throughout the past few years, I’ve taken a proactive role in finding out what is wrong with me.

After being stuck in Asia for most of the pandemic, and having pretty much a year on my own and locked down, I thought I was depressed and that this was the reason behind my low energy and constant tiredness.

I tried antidepressants which made no difference, then was referred to an ENT specialist who diagnosed me with sinus issues and a deviated septum. 

I then had a septoplasty and turbinate reduction thinking this would solve all my problems, I think it helped slightly but not all of them.

Last week I had an at home sleep study using the Alice NightOne and have been given a prescription to buy a Resmed Airsense 10 Autoset. (I’ll attach my sleep study results here).

I’ve now ordered this along with a F30 face mask and it will be arriving on Monday.

I really hope that this improves my energy levels and sleep as I’ve not got energy for anything and getting through work is such a struggle. I constantly get headaches and can easily sleep after being awake for 2 hours. 

Based on my sleep study, what settings should I start with on the machine? 

I’ve got an SD card ready and have downloaded OSCAR and will upload data when I have it.

Welcome to ApneaBoard!

Thanks for posting your sleep study.  It looks like you are diagnosed with Mild Sleep Apnea.

Hypopnea at 2.6, and Apneas at 3.3 (centrals - 2.1, Obstructives - 0.8, and mixed apneas - 0.4) with a total AHI of 5.9 per hr.

Remember that a sleep study is just one night, but does give us something to start with.

When you get your machine, the following settings would be a good starting point.

Minimum 7cm
Maximum 15cm
EPR - set to 3 (this will give you a exhalation of 4cm.)
 

Expect adjustments depending what we see after a day or two.

So my CPAP arrived yesterday and I used it for the first time last night.

It was much comfier than I expected it to be but had some pulling on the mask from the hose. Might consider getting a hose holder?

I still woke up a couple of times in the night to go to for a wee.

There was one time where I rolled over and had some mask leak.

I've attached my data, should I stick with these settings for another night?

I would stick with these settings for a couple more days.  Obstructives are low, Flow Limitation is low, and the leaks as you state are from movement. A hose holder is a good option.

Also, read the Mask Primer.  Some good information on mask fittings.

Your sleep study did show some CA's, so we'll watch to see if they increase or not.  If so, we can always lower the EPR a bit, but give it a few days.

One thing, work on eliminating the ramp feature if you can.  Every time you stop to take a break, you start over at 4cm which is too low and you're really not receiving much therapy during that time.

I think it was set to auto yesterday. Shall I try with it off?

Try the ramp setting on Off, unless you feel you absolutely need it (most don't with a low minimum pressure).

Don't forget to post another chart.

Here is the data from my second night of using the CPAP.

The first section of sleep is the longest I have slept interrupted in as long as I can remember.

Please let me know your thoughts and if you think anything needs to change.

This looks pretty good, and you slept longer so that's a plus.  I think you could have slept all the way through the night except for the clusters of Apnea around 2:40.

This is what we call "Positional Apnea".  You were sleeping in a way that was cutting off your air.  This can happen on your back or even side sleeping where you tuck your chin to your chest.  Also, be aware of pillow thickness.  If too puffy and tall, your head can tilt forward causing the same affect.  Use one supportive thinner pillow.  

I wouldn't change any setting just yet, but be aware of how you're sleeping.

That's good to know. I'll be aware of this from now on.

I'll keep you updated and upload more charts over the next few days.
