The medical community has long accepted that insurance (CMS) defines the conditions that qualify for treatment, and have diagnosed to that standard rather than using good medicine and optimal NIV devices and settings. This series of position papers may be a starting point to discuss changes in insurance policies to qualify for device, testings as well as compliance. No organization is more bureaucratic than CMS, so I won't be holding my breath.

I see  your point but a lot of pressure has to be provided to remove the inertia of Medicare/Medicaid and the Health Insurers. Hope this is a strong start.
