When you say for a few minutes, you mean while awake? It's unlikely the dynamic nature of VAuto kicked in while awake, but if you mean he fell asleep for a few minutes, then it could have.

The only starting difference in pressure between the previous settings and Sleeprider's advice is a 6/10 epap/ipap pressure versus 4/8 from before. If you'd like, you could keep Sleeprider's settings but lower the Min EPAP to 4, and the starting pressures will be the same as they have always been but still be able to increase after he falls asleep.

Hi Sleeprider  :Smile He slept last night with these pressures and it did seem to help a bit.  I will attach the oscar data.  He did find at these pressures that he was getting a lot of mask leaks up to his eyes and on the side of the mask even after tightening it.  Thank you again for your help!

It looks like from the oscsar data, that his obstructive events went down a lot, but his "clear airway" increased. What exactly is clear airway?

We need to cut back IPAP max from 18 to 16 and see if we can reduce the CA events. Otherwise, the obstruction is indeed cleared to a much greater degree. The pressure is rising on flow limitations that occur in 4 clusters. I suspect a positional issue with chin tucking, but for now the therapy looks good until pressure rises above 16, which seems to cause the CA events. Avoid the temptation to over-tighten the mask. A good fit is a mask that can float on the face. If the seal does not happen comfortably, the mask fit is wrong, the straps are not too loose.

Sounds good - we will adjust that.  So at this point do you think we need to push for a ASV machine or just stick with what we are doing currently?

Good morning  :Smile We just downloaded his night into oscar.  He is still at 12 AHI.  Is there any more adjustments that we could do to decrease the events.  We have ordered a dreamwear nasal mask in hopes that he will sleep better with it and not have so many mask leaks that wake him up.  Thank you!

Try reducing the pressure support from 4 to 3 that may reduce the number of CA’s

You asked: What is Clear Airway?

Clear Airway is an Apnea (10+ seconds 80%+ reduction) that occurs without an obstruction.  It is the same as Central Apnea, and is a part of Mixed/Complex/Treatment Emergent Apnea.

Fred posted the wiki article "Justifying Advanced PAP Machines" http://www.apneaboard.com/wiki/index.php...P_Machines . I want to draw your attention to the statement in Path 2 of that article:
"The new member that has been advised they have central or complex apnea needs to be aware that optimizing may provide significant immediate relief from symptoms and improve AHI, but improved therapy results can disqualify them from higher levels of therapy (ASV), that might be possible without this self-optimization. "

Jaswilliams suggested you reduce pressure support from 4 to 3, and I agree this may reduce the central events; however it may improve results enough that a doctor or insurance will not recommend ASV. Your husband continues to have CA events over OA events at a ratio of 4 to 1 and would clearly benefit from ASV therapy. We can optimize your results and reduce the number of CA events by cutting PS, however you have been warned why we have not done so.

Re-read the linked article and let us know where you stand with the doctor in discussing a move to ASV. Please minimize the monthly calendar on your charts by clicking the triangle in the date line.

That was a good point about the PS I did not make it clear with my suggestion why I was looking at reducing it but Sleeprider did.
