Holydoc - Therapy Progress

RE: Holydoc - Therapy Progress
Red,

I am on a Windows 10 machine (64 bit).  SD card contents were zipped and uploaded to your Dropbox.

Items that are incorrect on OSCARS display of my Device Settings:

Humidifier Status - Off   <---- this should be On
Patient View - Off  <-- this should be Advanced

The rest appears correct.

I hope that helps.

RE: Holydoc - Therapy Progress
Well here is testimony that you can have a really good report with OSCAR and still have a terrible night.

It seemed I would get to sleep and suddenly my mask was leaking all over the place waking me right back up.  You can see that my CPAP at 22:17 just kept shooting for the stars with pressure until it literally blew it off my face.  I had to take my mask off and readjust.  My CPAP continued to rollercoaster up and down causing the mask to leak and wake me up to adjust.   So even though this is the lowest AHI I have ever had, the sleep was one of the worst.

A few items of note.  I had cleaned both mine and my wife's CPAP that morning.  We had spent most of the day in our pool.  My nose congestion was nearly gone.  In case anyone asks, NO I did not submerge my AirTouch F20 mask.  I only wiped it off.  The other CPAP items were thoroughly soaked, cleaned, and left to dry for several hours.

Here are my OSCAR reports.  If there is anything I can do to NOT have a night like last night again, please advise.  Also if you want me to zoom in at a different area, please do not hesitate to ask.

Can you provide a zoomed in screenshot of around 11:45 and around 3:47?
That is when you pressure peaked.
Not sure why though, no obstructive events and no flow limitations either.
A zoomed in chart may show something.

RE: Holydoc - Therapy Progress
OK, I received your SD data.  In looking at the STR.edf file (the file that stores all this type of data) it seems that the file is reporting correctly.  When the AS11 debuted, Resmed modified some parameter settings for On and Off.  I think this one might be incorrect in the OSCAR code.  I will have to preform additional testing to verify this, since the reporting value seems to parsed in binary by Resmed.  They seem to signal on/off by setting the 7th bit in a binary value (010001=off; 1100001=on).  The reason I'm explaining is in case this might come up on Jeopardy or in a conversion at a cocktail party. Red,

I am glad I could help.  Hopefully it will be an easy fix in the OSCAR code.

Doug,

I have to apologize to you.  I posted my wife's screen grabs instead of mine.  OMG!  I am so sorry! She is a bit computer challenged so I been helping her post hers as well.  I guess I AM a bit CHALLENGED too!  So sorry!

What I typed was correct.  Here are the correct screenshots!

I would be in so much trouble if i said my wife was computer challenged, but she still uses the old AOL browser where she pulls her name from a list and it still says, "You"ve got mail."

RE: Holydoc - Therapy Progress
Luckily she is the first one to admit that she is computer challenged, so I can stay out of trouble.  I am very proud of her for being able to post on these boards as well as being so interested in OSCAR output.  I told her I would post her OSCAR outputs if she would type her questions and reply to her posts.  She has been doing well.  I am also proud that she asks me every day about what her OSCAR report says and what it means.  I am good at showing her the report, and thanks to you I am getting better at explaining what it means.  Now if I can just check to make sure I am posting the RIGHT screen grabs, I will be ok.

Thanks for looking at our reports and helping us.  This is all new to me, and my wife had never gotten any guidance since she started CPAP.  This has been a blessing to us to be able to make adjustments that actually make a difference in our health.

RE: Holydoc - Therapy Progress
Just a quick comment about your sleep. I like the gentle breathing after 1:20.
But your flow limitations are too high, however the tool we use, EPR is maxed out already.
You can raise your minimum a bit to head off these flow limitations.
Try 11 cm for a few nights and bump it to 12 cm if you have no issues.
This might treat the flow limitations and stop ResMed's aggressive pressure increases to deal with flow limitations.
Raise your ramp to 9 cm if you can, extend ramp to 10-15 minutes to give you a chance to fall asleep without pressure increases, if no problems raise your ramp to 10 cm in a few days.

if you can't get the flow limitations under control, you might need a bilevel machine, I had to do that.

RE: Holydoc - Therapy Progress
Doug (I hope it is Doug),

I am going to leave my machine at its current settings for tonight to try out a different mask.  My wife had a F40 that she hated that she is "letting" me try.  I think I will give it a try since it will be much less intrusive than my current F20.  Her thoughts are that I might just need to switch it up a little to give my face a relief.  It is possible that I just need a bit of a mask break to get me away from my current leak fest.  LOL

I just don't want to change too many variables at a time since I will get easily confused on what is helping.   Thinking-about  If it works out well with this mask, I can make the changes you suggest above and see how they help.  I am thinking that you are definitely getting me dialed in with the right machine settings.  Now I am venturing into the area that bothers everyone.  What is the best mask?  I do like my AirTouch F20, but I do not like that it has a tendency to leak at the cheeks (farting sounds) that wake me up.  Also we found out when we went on a trip, that I had to pack my mask separately than my CPAP machine.  It would be nice to be like my wife and be able to place the mask in with the machine.
