Low AHI but still feeling fatigued most of the day
Hello all,

Some things to note:
  • I was diagnosed with severe obstructed sleep apnea (AHI of over 69 during the study)
  • Significantly deviated septum/congestion (sprays/salt water does not help)
  • Mouth breather (practicing breathing through nose)
  • Takes 40 minutes or longer to fall asleep.
  • Experiencing fragmented sleep for around 5-6 hours, wake up, sleep again for 1-2 hours, repeat
  • SpO2 % Levels: mid-to high 90s
  • PR(bpm): 70s to 80s
  • AHI has consistently been low single digits
Unit: ResMed AirCurve 10
Mask: F&P Evora full large

Current settings:
Mode: VAuto
Min EPAP=10
Max IPAP=25 (Typically doesn't go above 18 in practice)
PS=6
Trigger Sensitivity: Very High
Humidity Level: 7 of 8
Humidity Temperature: 83°F (85 max)
Ramp Time: off

Tried different settings to no avail.
Mostly central sleep apnea, anywhere from 10-40 seconds per clear airway cycle.
OSCAR data resembles Cheyne-Stokes respiration.

Going to see a better sleep doctor hopefully within a month.
Are there any pointers/settings I can try to obtain better sleep?

If you're already using the Trigger setting at Very High, then let's try lowering the PS to 4 in hopes of bringing down the CA's.

Did your sleep study show central apneas?

You also have some masks leaks that need attention.  Be sure the fit is right, and that the cushions don't need replaced.

Yep...I agree with everything Opal just said. Just going off the one night you posted it almost looks like you could drop your EPAP to...say...9 and see if the lower pressure helps with leaks too. You're currently set at 10 and it never got above 10.38 so you could probably drop to 9 or maybe even 8 and be okay. Your humidity seems high but that's probably because mine is set at 4 but if you need it you need it.

Thank you for the response.

I lowered the PS to 4 but was unable to sleep so I reverted it back to 6.
My sleep study displayed mostly obstructive apneas with some hypopneas, but my CPAP titration was insufficient and displayed 212 apnea and hypopneas 
(46 obstructive, 75 centrals, 91 hypopneas).

As far as mask leaks, I ensure that the mask is secure and that no air is leaking. I've read that an acceptable number for mask leaks are 24l/min. 
I've attached data from the last three nights.
My data shows that most of the time it's under that, and this is true for everyday.

My AHI has been hovering around 1-3 for the entire month.
5/22 is when I tried PS4. AHI went up to 4.66
5/23 it jumped to 5.62
5/24 went back down to 2.29

Is there any other information that might be useful, such as Mask Pressure, Tidal Volume, etc?
Also, I'm using a BiPap machine and read that I might actually need an ASV machine to help alleviate central apneas.
