Do you want us to know when you die? [Apnea Board Postmortem Form]
Let's face it, we're all going to die someday. To many here, our friends on Apnea Board have become like a family. We have numerous active members whom we've grown to respect and admire. This past year, two of our forum staff members (
JustMongo &
PaytonA) have passed away. When they passed on, we here on Apnea Board didn't know about it right away - they simply stopped posting, and we were left wondering-- what happened to them?
So, at the request of Apnea Board Monitor
PollCat and our Advisory Members, a notification-of-death form has been developed for you to use (if you wish).
Advisory Member
srlevine1 has created an "
Apnea Board Postmortem form" in PDF format. You can download, print out this form and fill in your forum username (screen name) in advance, so that when you pass away, your loved ones can send the form to Apnea Board to let your friends here know of your passing.
The basic form can be downloaded here:

A separate form with "editable fields" can be downloaded here:
http://www.apneaboard.com/Apnea-Board-Po...e_Form.pdf
Thanks to srlevine1 for developing this form and to Crimson Nape for the editable version !

Just wanted to let folks know that
Crimson Nape made another version of the form that has "editable fields"... see the links (now updated) in the post above this one.

Also, if you'd like to use it, Advisory Member
Sheepish created a pre-addressed envelope document that you can print out (with Apnea Board's mailing address on it).
You can download that document here:

Well...that's a thoughtful idea, and I like it. I am also quite active on some hobby forums, and about twice a year someone posts, "Haven't heard from So-and-So for some time. Anyone have anything to share?". Only then does another person report that the person passed months ago. Others figure out whom to contact and learn of a serious turn of events of some kind or another. It's sad. Those people all had personalities and life's experiences that they shared, and they were almost always engaged and engaging.
Nice touch.

One familiar died. His FaceBook account survived. It wasn't fun when she got her birthday congratulations for years. Now a memorial account is possible on FaceBook.

I was feeling fine until the bump.

Great idea.
Would it be appropriate to send a PM to the membership with a brief description and link to this thread? Many will probably not know this exists otherwise.

People may also wish to arrange for executors to have access to the password lists on our electronic devices and include a request in our wills that they contact every one.

I am sure many people do not know about this. The thread does not appear in the main forum and I clicked on the link out of curiosity having misread the title as "Do you want to know when you die?" I do agree that the forum members are very friendly and there is a "family" feel to posting on the forum.

Interesting idea, after spending a day with my wife and lawyer working on an Estate Plan I had to spend some quiet time alone trying to forget the reason I had to do this.
If anyone commented on my passing, it would be nice to be around to read what was being said.
This leads to another thought, the recall. You have been here from the very beginning. I feel like I know you for at least a year. Many fellow members have gone through very hard times during that time. From financial problems to medical problems to emotional problems.
Maybe having a thread where people can post their own individual stories, or just vent out their feelings, without any feedback would be something to consider.
