Ok, so higher sensitivity will initiate an inhalation from the machine quicker and maybe resolve the Central apnea events. Worth a try!!! lol. It’s been really hard to get to sleep with them.

Sorry to add so many posts. I looked and my machine is set on medium. Wondering if I should just go up to high first to see how I react to that before I try very high.
But if you all think it’s the very high setting that will make an impact I’ll go to it

Since your problem is central apnea, go ahead and set to Very-High. Where there is uncertainty, I usually change from medium to high, the very-high. In your case, I want you to feel confident the device will provide a breath rather than keep you waiting. The mechanism we are using is your own spontaneous respiration that we hope to stimulate with a pressure change. In many cases this works well to solve the problem.

I did not see this message until this morning!
But even with the high adjustment, I slept better last night and had less events. I was down to about five AHI still, most of them were Central. So I will try the very high setting tonight! Really appreciate all your help on this and I’m wondering why doctors don’t think about this setting more often?? If this works and I don’t have to go back in for another sleep study and pay for another more expensive machine I will be very happy lol thank you again.

Glad the setting edit helped. You likely wouldn't benefit from any new sleep study. Most, if not all, new sleep studies are to generate more billing to pay for boats, not really to benefit the patient. You have one sleep study report that ought to suffice for your therapy requirements. Your insurance will not request these.
You have your copies of the detailed sleep study report, detailed Titration if done, and the script? If no, request each ASAP.

I believe I do have some detailed reports for my sleep study. I’ll have to go into my portal and see what they sent me. I’m also still working on a way to download Oscar.

As I mentioned on the last page, the doctors and suppliers seem unaware of the sensitivity settings for trigger and cycle, or don't know how to use them. I'm fairly certain we can move you to a lower pressure once we work this out. The Very-High setting should help with your therapy and feeling of suffocation as you're falling to sleep. It's an interesting sensation as you become aware you're not breathing, the machine suddenly provides an assist and you remember to inhale. It's not as foolproof as ASV, but it often solves a problem.

Will report back! lol. Last night was the lowest AHIs I have had since starting.

Well, you guys were right, raising the trigger sensitivity to very high resulted in 4.7 AHI per hour and with a flip-flop from what it’s normally been- 2.6 were obstructive and only 1.8 were Central!!!
However, I still had a really hard time getting to sleep and getting any good quality sleep last night. I still felt that I had a couple weird events trying to get to sleep, but they may have been more coughing and throat issues then apneas. And according to the Fitbit, my HRV dropped from where it usually was, and my resting heart rate was up a couple points from where it had been overnight usually. I did not wear my oximeter in my finger because the more stuff I wear the harder is to sleep lol I will wear it tonight or the next night once I keep getting used to this Anyway, this looks like progress, but I still need to get myself on track to sleep better! |
