I think you misstated your question. I think you meant EPR and not EPAP.

First now focus on comfort. Your numbers are good.

With Central Apneas present good results are always a balance between obstructive and Central events. At this level IMHO Central apnea is not as bad as obstructive.

That said try EPR=2 and assuming no large changes see which feels better. Small changes. If you were on a BiLevel I'd suggest a smaller change.

Hi,

I think I'm nearly there, thanks to you guys.  I am using cervical collar, and following all you recommendations, and would like to see if there are any other adjustments I could make.  I also feel ok, making it through the day.

This looks like an effective range of pressure for you and there is relatively little pressure fluctuation. If you tolerate EPR, increasing to 3 might be beneficial to further reduce flow limitation. With 95% FL at 0.09 you are in an aceptable range, but it might be more comfortable and reduce those frequent inspiratory spikes we see.
