OSCAR/HR Data Review Help - Possible UARS
Hi all,

Male, 30s, 5'7, 165, physically active with gym/cardio/rec sports

I'm new here, recently discovered this forum via Reddit, and am hoping to get some opinions on my sleep situation. I've been troubleshooting persistent night sweats and sleep disruptions since 2020, and recently began using OSCAR and CPAP therapy more seriously (within the past few months). I have no issues using a CPAP for the entire night at this point, it just does not seem to be helping. Previously, I was masking my main symptom, night sweats, with glycopyrrolate, but the side effects became intolerable. I'm hoping someone can review my OSCAR data for a few nights and provide me with a better interpretation than ChatGPT. 

I have a low AHI most nights using CPAP (average <1), but still wake up 2-4 times per night, drenched in sweat to the point where I have to lay a towel down or switch sides of the bed. These sweat episodes typically correspond with REM cycles. I have tried a ProSomnus oral appliance with no luck and most other basic methods over the years, like sleep hygiene, positional therapy, mouth tape, etc. 

I have 10 nights' worth of data so far from OSCAR and my Garmin watch, which tracks (poorly but using for trend analysis) HR and Sleep Stage, and am logging each night's symptoms and relevant information in a journal. I've attached 1 night of OSCAR/Garmin Data because of my New Member Status, but I'm happy to provide more once I'm able to. I woke up sweating 4 times this night.

Sleep Setup:
Machine: ResMed AirSense 10 AutoSet
Mask: ResMed DreamWear Full Face
Mode: AutoSet (Soft Response)
EPR: 3 (Full-Time; previously tested EPR = 2 with no improvement)
Pressure Range: 10-15
AHI: Typically between .3 - 1.5

Did you have a sleep study? What did it say happens while you sleep?

There’s a lot of reasons you could be waking up at night sweating. Have you talked to your doctor to see what else besides sleep apnea it could be?

Yes, I've had a few at-home sleep studies, which showed AHIs of 5.4/8.8 and RDIs 8.7/17.6. At this point my insurance won't approve an in-lab study, but I've continued to push for that.

I've been tested for a variety of possibilities (disease, infection, cancer, medication side-effect, etc.) by my PCP, but all were dead ends. I will be taking a few weeks of OSCAR + Garmin HR Data to my sleep specialist on my next visit, but the Internet has led me down the UARS rabbit hole so thinking it might be that. 

Adding a few extra OSCAR screenshots in case that is helpful for assessing my situation.  Appreciate any advice or opinions!

Basically no residual flow limitation getting flagged. I’d expect you to feel pretty good with graphs like this. Have you ever tried filming yourself while you sleep with a cheap smarthome camera? That might give other clues.

This looks pretty good...do you mind giving us a few examples of individual waveforms (for flow rate, give us a zoom of about 2 or 3 minutes).

I have a 4.5 AHI with 4% desaturation but a 30 AHI with 3% desat, which is "UARS-y". You can look at some of my charts. Lots of big spikes constantly.

It seems like you have some sort of sleep disordered breathing, but I'd really push to get an in-lab (as you seem to be doing).

Night sweats could be indicative of a heart issue. 

But if you have had previous testing, surely it would have revealed this.

Thanks all for taking a look. 

Haven't tried a camera but have recorded for sound and don't hear much. Get yearly EKGs due to family history and nothing's flagged yet thankfully. I have slightly elevated BP so on medication, but night sweats pre-dated this. 

I've provided a few screenshots at 2-min intervals. Let me know if not what you were looking for. I've also uploaded a chart with some of my flow-rate/nightly data. I've tweaked my settings since these nights to EPR 3 and Min 11 and believe I am starting to see improvements but not sure yet.

EPR: Full Time 2 cmH2O
APAP
Min 10.0 Max 15.0 (cmH2O)
