Yes, you definitely need an ASV.  Ask for a Titration Study including the use of an ASV.  That should make your doctor realize that you need the ASV.  The prescription can be written by any kind of doctor, NP or LPN.  You don't need a sleep specialist.  Lobby for yourself to get what you need.

In the meantime, to try to get a little more comfortable, drop your ramp time to 10 minutes, raise the ramp pressure to 10, and try a narrow pressure range.  I suggest 13 to 17, as you are using 13 now, and you need a little headroom.  Try it, see how you feel, and report back. We'll help you get as comfortable as we can until you get the right machine.

Thank you so much I really appreciate it! I dropped my ramp to 10 and set my pressure to 10-14 and turned off EPR, still feel like I got really poor sleep unfortunately. here is my data; it does look like CA went down but unidentified apnea went up

You left your machine in cpap mode, set at 14.  It would be better to put it in autoset mode and set the pressure range at 10 to 14.

You had a lot of leaks last night, unlike the other nights.  That might have contributed to you not feeling any better.

It may seem counterintuitive, but since your machine cannot fight the CAs anyway, I think you should turn your EPR on full-time, set at 3.  Your AHI will likely rise, but at this point who cares?  The EPR 3 will make your breathing more comfortable, which I think is more important.

I'm praying that you will soon be able to acquire the ASV you need.  Smile

Yes, leaks are an issue. Ramp also is going to hinder still, and it likely will add variability that may increase CA. I would consider taking Ramp off.

Certainly try EPR and an AutoSet pressure range. Get the most comfort available until you convince the doctor to order ASV.

As much as possible, keep bugging the doctor you need ASV. Don't let them forget you. Complain till their ears bleed, being serious.

Last night was a lot better! another major change I made was filling my humidifier tank to the max level; typically I fill it to halfway and it always drains all the water and this morning it looked like it drained almost all of it with some still left over. Could humidity be a cause of any clear airway events?

I would be inclined to say humidity isn't a factor with CA. With high levels of predominant Central Apnea (also is CA and flagged as Clear Airway, all pointing to same thing), this is probably idiopathic medically, meaning unknown cause.

It's not that humidity won't affect therapy feel, I'm not sure it'll affect CA directly.

I agree with Dave on the humidity.  

Look at your OSCAR chart and see how "vertically" Leaks line up perfectly with your CA's.  If you can lower leaks, the CA's should decrease.
