Please help me lower my AHI
Been using AirSense 10 for more than a week now but my AHI still is high. My doctor increased the pressure yesterday to Min 7.0 Max 14.0. Unfortunately, it did not help much as my AHI last night was even higher than the previous nights. Btw, my untreated AHI was 29.8, so borderline severe. I'm 24M, 5'6, 65 kg.

![]() Your pressure range is too low at both ends. Your median pressure is over 11, and you are bumping against your upper limit too much. You would do well to set your range to 10 to 20. Don't worry about the 20. Your machine will not go higher than you need. You show a lot of CAs. Did you have a lot in your sleep study? If you're not sure, please post a redacted copy of the study. If you don't have a copy, ask your doctor's office for one. Hopefully, they will provide one, or maybe ask them to tell you the percentage of CAs you had. You also show a lot of Positional Apnea. PAs show on your chart where you see clusters of obstructives and/or hypopneas. No setting will fix these. They are caused by your chin dropping toward your chest, and cause air restriction, much like a kinked hose causes water restriction. Some can fix this by sleeping on a flatter pillow. More people need to wear a soft cervical collar that is high enough to help keep their chins up. Most drug stores carry a few, and Amazon carries lots of them. Lots here like the Caldera Releaf Collar because it is very comfortable, but some find they don't go as high as some folks need. Hi! Thank you for your response. I checked my sleep study and no, I did not have a lot of CA's. I attached a copy of the sleep study, and also a report from the previous night. Strangely enough, I didn't have a lot of CA that night. I think it increased because my pressure was also increased yesterday?
Thank you about the info about the positional apnea. I totally had no idea about that. Will definitely check out the Caldera Releaf Collar ![]()

Your pressure is not correct. Your machine is flailing around all night trying to increase itself.
What I would suggest is either: -an in lab titration, where they find pressures that eliminate your events or -an online company that will work to titrate you up to the appropriate therapy I've been doing the latter and it is not that expensive and will get someone helping you who has done thousands of titrations

Your pressure settings are way too low. Your median pressure is a little over 10, and you are bumping against your upper limit way too much. Try setting your pressure range to 9 to 20 for now. Don't worry about the 20, as your machine will not go higher than you need. This should help you sleep better and more comfortably. Give it a try!

Looks like you were mouth breathing.
Your therapy was compromised with such a large leaks, Nasal pillows kind of magnify these leaks. Do you use a 2x5 strip of mouth tape, or I like a SomNoSeal, an oral sealing device that encourages nose breathing and eliminates mouth leaks and dry mouth during sleep. You have clusters of Obstructive events that are probably positional apneas, where you are tucking your chin to your chest and cutting off your airway, like kinking a garden hose. No amount of air pressure will open your airway up, you have to move, roll over. Consider using a soft cervical collar and until one comes, try using a small pillow held under your chin to prevent/minimize this tucking. You need to raise your ramp pressure to 7 cm or turn ramp off if you are comfortable doing so. |
