Having trouble finding the right pressure settings
Hey guys,

I started APAP roughly 10 days ago after being diagnosed with 23.5 AHI, I now am regularly under 5 AHI but I still feel tired, I guess my sleep is still disturbed and I would like your help understanding why. I played a bit with the settings so I will first post a very regular night so you can have a general picture of my case  Smile

   

I then tried to make the pressure range narrower, but it didnt work that well. 

My symptoms are : (daytime) brain fog, yawning a lot, not having energy, my ADHD meds not working well as if there was a new wall of fatigue it had to go through. nightime symptoms (with apap) are : I feel like I am about to cough/suffocate a lot, bloating/aerophagia, an urge to breath through my mouth.

My mask is the resmed N20 medium size

1. Your Min pressure is too low.  Look for the pressure settings on your Sleep Study.  If you don't have them, ask your supplier and/or doctor.  If you still can't get them, you will have to find your optimum Min pressure for therapy. Min 7 is a good place to start.  4, as you have, is not considered effective for adults.

2. Your large number of CA events could be due to adaptation to therapy.  Check if CAs were major on your sleep study.

With a bit higher Min pressure and adaptation, your Flow Limitations and various events may lessen, which will bring down the AHI to an acceptable level.

I will try 7 as the minimal pressure then, CA were not a problem during the sleep study so I guess it comes indeed from APAP adaptation process. Is 12 a good place to start for maximal pressure or can it be too much ? Thank you very much for your help, appreciate it

12 is not too much.  Your machine never went to 10.  12 is doing fine.

Hi again, I tried your settings and I feel a bit better so thanks again, however my numbers are still a bit messy, should I keep them the same for a few days before trying any changes ?

Most of your events are CAs.  Those are typically not treated by air pressure.  I would leave the Min pressure for a few days to see if the CAs reduce, then maybe try 7.2, 7.4.  You are fortunate to need so little pressure for therapy.
