Resvent Ibreeze 30STA settings

Resvent Ibreeze 30STA settings
Couple weeks ago I bought Resvent 30STA from MBR Medicals. I replaced my old Resmed Airsence 10 for her. My settings for Airsense was Pressure 6-9 and pressure release mostly 3. on this settings my AHI was range of 5 mostly CA events. Changing pressure release to 0 didnot helped to much  with CA or periodic breathing.
I was not able to get clinical manual for 30STA, so setting was more chalenging. After over one week different try currently I am using machine as :  Auto Spontanious (S) mode,  IPAPmax-9, IPAPmin-6, EPAPmin-4, EPAPmax-6, rise time-500ms, Vt-590ml, Press Adj-2(I dont know what it is?), Timin -0.9s, Timax1.9s,  Itrigger and Etrigger-Auto.   Itrigger and Etrigger can be set Auto or 1,2,3,4,5.   I dont know what is moust sensitive triiger 1or 5, I assume 5 but not shure.
On Airsense my tidal volume was ussualy about 400ml, on 30STA is close to 600ml. I think callibration of flow sensor on Resvent is not correct, I rather trust Resmed, but it is only relative number for me not to much important.
With this settings: Epap: 4-6, Ipap: 6-9, and Vt-590, PS changing beetween 2 and 5.
I fill more comfortable and better sleep with this setting then if I set constant PS. I can set constant PS only if I dont set constant Vt. With constant Vt PS is veriable. AHI is usually range of to what is better then with Airsense, and also I fill more comfortable  and can better control flow limittation and leak.
Resvent Ibreeze 30STA has software version: V01.14.00 and machine has been made in 2022.
I know not to many peaple use Resvent machine and probably even less settings like my.
Asking for opinion. my be some advise.
Sorry for my English

RE: Resvent Ibreeze 30STA settings
You are probably not able to download and use OSCAR on this particular machine.  Correct?

A few things:  check to see what all of the different modes are (you are currently in S mode).

Your epap (max and min) looks a little low.  How does it feel during sleep?

Ipap may be a bit low as well.  Again how does it feel?

590 tidal volume is a bit high.  For an average size male, around 500 is around normal (can vary though).

The Ti min, Ti max are timing controls for inhaling and exhaling.

Trigger is the setting for the back up rate to start.

I have used several ventilators.  The Resmed ST (A) was my 1st machine.

Describe anything that feels off.  Like not enough time inhaling, exhaling; too much time, etc.  Too much pressure or not enough pressure, etc.

A couple of months ago my RT from DME removed the Philips Evo ventilator I had and replaced it with this Lowenstein Luisa ventilator.  After some tweaking, it seems to be functioning very close (if feels identical) to the old Evo.  I assume your Resvent may function similarly to the Resmed ST (A) if the settings are close enough (and all of the timing controls, etc.).

May take some time and tweaking.

Most important thing about ventilator set up:  if you ever feel any pain in your chest or lungs, lower the pressure(s) and maybe also lower the back up rate and Vt. if you are using them for your own safety.

RE: Resvent Ibreeze 30STA settings
Thanks for responding.
Tonight I will increase Epapmin by 0.5 to 4.5. Resvent machine showing my tidal volume  higher then before RAirsense 10. I think 590ml on 30STA is equal about 420ml on Airsense. Because of this my setting tidal volume for Resvent is so high because I think of different calibration of flow sensor. I will try to attach some screenshoots from last night.

RE: Resvent Ibreeze 30STA settings
Thanks Red for the post!

Antoni, as few major things stick out from these OSCAR charts.

1st - not bad therapy.  Overall, charts look pretty good IMO.

2nd - you have a pretty high minute ventilation rate.  Minute ventilation = respiratory rate (18) x tidal volume of 590.  Normal rate is around 12 to 20 per minute, but can vary.  Tidal volume is acceptable.

3rd - high leaks at times.  Here are some tips from our wiki:
  • 1Mask Primer
4th - it looks like right when you fall asleep, your tidal volume, etc. increases dramatically.  But then they slowly start decreasing back down to more normal levels.  Are you aware of this?  Does anything feel out of the ordinary when you fall asleep?
5th - as long as you are feeling ok, these settings are probably pretty close to optimal in this mode.
I will try to post the Resmed titration guide for the ST (A).  It is probably very similar to your Resvent STA.  You are in S mode now.  That is spontaneous breathing mode.  The machine lets you breathe when you want to.  Good news, with an 18 breathing rate, you don't need the timed mode (has a back up rate if your breathing rate is too low).  And the Resmed ST (A) has an Ivaps mode as well (it is mainly for patients with COPD, Neurological disorders, etc.) to keep tidal volume high enough at a certain level with every breath all night long.  More good news:  with your 590 tidal volume, you don't need this mode either.

So most likely stay in "S" mode.  Then possibly some minor "comfort" tweaks if necessary.

EDIT:  Sorry.  I see you are in "Bipap" mode and not S mode.  You are right.  For some reason the manual is not available.

RE: Resvent Ibreeze 30STA settings
                Thanks Jay51 for response.
Resvent Ibreeze 30STA has total 7 settings:
CPAP,
S{30},
Auto S{30} - what I am using,
S/T[30}.
T[30},
PC.
With firmvare V01.14.00 I did not find any abnormal behaviour of machine, except that measurment of flow sensor is to high, but I accept this even easer if I compare price with Aircurve ST-A which nprice is 4 times higher then my new Resvent 30STA.
Regarding leak I am using P10 nasal pillow mask and chin strap. It is no leak when I sleep on my right side. When I move on back I assume that sometimes I sleep with open mouth and then is leak. Chin strip not control this 100% and I also not trying to tight it to much. I use to try mouth tape but I had bad expierience with tape and I will not try it any more.
I dont know how to call mode in what I am using machine, because it is not typical as is non typical machine.
Display shows : Auto S{30}.
Setting: Epapmin-4.5,  Epapmax-6
            Ipapmin-6,  Ipapmax-9
Vt-590ml (and it is controversial number because its really mean about 420ml in Resmed Airsense 10 which I use before)
Ti (Inhalation time): 0.9s to 1.9s
             Increase inhalation time  - 500ms
Triggers for inhallation and exhalation in Auto (option are: Auto, 1, 2, 3, 4, 5) I am confuse whitch one is most sensitive 1 or 5.
In this mode PS is veriable fluctuating beetween Epap and Ipap and PS changing from 1.5(6-4.5) to 4.5(9-4.5).
Last night sleep was little worse then before because insomnia, but not bad. I fill comfortable with current settings, more comfortable then with previous Resmed Airsense 10 machine.
In future maybe I will try to make small adjustment, specially with Ipap.
I am not shure if I can do something with hypopnea and CA events. They looks like periodic breathing and accure ussually during transition from awake to falling asleep and oposite and I think are connected with higher leak also.
But I prefer my body and mind deal with this alone instead to force it to higher more restrictive settings or mode of machine

Jay mentioned AVAPS style Titration. Here's 3 images of the ResMed guide. Maybe this helps out. The original page images needed split to fit the attachment requirement.

RE: Resvent Ibreeze 30STA settings
Thank you again Antoni for providing that.

The titration guide for the mode that you are currently using is pretty simple (you have just about figured all the settings out already).

You probably chose the best mode to begin with (Auto S).  Great job.

This time in your charts I see some more H's (and few more CA's - not that big a deal).  For H's titration protocol says to raise IPAP until they are controlled.

You might even get a little relief from this article on positional apnea from our Wiki:

[*]5.3Positional Apnea

Things like lowering pillow height, side sleeping instead of back sleeping and keeping your chin from tucking towards your chest, and even a soft cervical collar might help.

If the H's start to cluster together, then use more positional apnea techniques to help prevent them as well.

With my old Evo ventilator, I remember the lower the numerical setting, the less it would respond.  So with 1,2,3,4,5 one should respond slowest, etc.  You can try these if you want.  Factory default is usually in the mid range (probably 3).

If you start to get more CA's, then T mode would stop this.  Timed means a timed back up rate to force you to breathe every few seconds.  But you are doing ok in Auto S mode IMO.

This Resvent may work out for you.  Therapy does not look bad.  If it is comfortable, then great as well.  Keep posting OSCAR's and we can keep tweaking and find tuning.

Thanks Dave for posting that.

RE: Resvent Ibreeze 30STA settings
Thanks Jay and Dave for your support.
Yestarday I increase Epapmin by 0.5 to 4.5cm.  I would like to spend couple nights on current settings without changing anything to see how it works for me. I would haracterize this setting as BIPAP veriable (because PS is changing) and spontanious.  Earlier more then week ago I was trying in the same mode Auto (S) different setting,  I would call it true BIPAP, I switched off setting for Vt, so I was able to set constant PS. I tryed PS = 3 and PS=4. I gave up with this setting, because worse feeling in the morning and also worse indexes of CA, H, Fl. May be next week I will increase Ipap by 0.5cm to try.
Also in next future if I have to change something I would try to go to next mode, what is called Auto S/T(30). This allow to setup backup breath when pause in breathing is longer. But I am not shure about this.
I know my breathing is rather lazy and shallow. I am in some advanced age, but medium built, no overweght, healthy according to age and physically active. Regarding IVASP I think it is not yet time for this mode for me. I would be scare to setup this mode, not to screw up  balance of CO2 in my blood and consequences of this. I dont know I am right or wrong, this is my way of thinking.
My former sleep doctor did not want proscribe for me better machine then Resmed Airsense Autoset. After one night sleep study he wrote on  proscription setting CPAP=7 or APAP=5-10cm.
Because of this I bought Resvent 30STA machine from my pocket.
For next sleep study and different sleep doctor visit I have apointments in May. I am thinking would be good to get Resmed Aircurve VAuto.
In Ontario insurance cover 75% cost.
