Yes I think you are right. It is frustrating because I was diagnosed with specifically severe obstructive sleep apnea when I only had 6 obstructive events in my sleep study, when my events were primarily central. Maybe the high number of hypopneas influenced this diagnosis. I just want to feel normal again. I guess I have a long road ahead of me. 

Also, not sure about my theory about kratom, maybe it aggravated my underlying central apnea but I’ve been off it for 5 days now and still having large numbers of centrals.

Unless you address centrals as a hindrance to sleep and gaining well rested state, docs mostly are blind to Central events. Remedy? Discuss with doc what the centrals are preventing. Fatigue, not treated, not rested, etc. Take OSCAR charts if you want, but the most weight is on your symptom and complaint presentation. State the current treatment isn't acceptable and that your research into ResMed machines tells you only an ASV treats Central events and that's what you request.

The clinics don't usually make an effort to determine if hypopnea is central or obstructive, and where a determination is not made, always assume it is obstructive. Nearly all initial diagnoses are for obstructive apnea, even in cases like your where they are clearly predominately or entirely central. Whether this gets corrected in a subsequent evaluation depends upon whether the patient survives the poor therapy results without quitting, and whether the doctor is both knowledgeable and willing enough to revise the diagnosis, knowing it is a much more difficult treatment.

Consider the doctor's motives and commitment to improve your results. Many sleep specialists do this for extra easy income, secondary to their primary specialties. They mainly review and sign sleep technician reports and write prescriptions. Many of them never actually meet the patient. The highest yield for the least effort is to routinely diagnose obstructive sleep apnea and get referral fees from the DME suppliers. Unless a doctor has multiple patients being treated for central apnea, they are a lost cause for you, and that needs to be the first screening questions; do you treat other patients with central and complex sleep apnea with ASV? If your doctor cannot answer this with an enthusiastic yes, you're wasting your time with him. You need someone that routinely diagnoses and treats this in their practice, otherwise you are in a "production apnea shop".

RE: Second night on APAP, lots of Central Apneas, is this normal?
This is the first time i've seen CSR breathing in my results, should I be concerned? I just had an echocardiogram done 2 months ago and heart health came back perfectly fine. I did set my pressure down to 5.4. still barely any obstructive events.

That is not CSR. It is periodic breathing during a brief time when your respiratory drive oscillated. This is usually just a fluctuating pCO2 caused by a feedback loop of the increased and decreases ventilation in the cycle itself. Nothing to worry about.
