I felt very much the same as you when I first started therapy. One exception is my overall AHI didn't go down. Regardless, my fatigue didn't reduce. The key to success was to keep getting tests, exams, and etc. until a full health picture was created.

For myself, my apneas weren't the standard normal obstructive sleep apnea, but were mixed, as in obstructive and central apnea combined. This was only revealed by additional sleep studies.

For you, discussing the issue with your doctors and digging deeper into your full health situation is likely the best thing to do, IMO. AHI numbers are never the whole picture of apnea treatment success. If you're not getting rest, treatment is failing you. But that fail doesn't mean CPAP is not helping, but most likely means it's not enough on its own. Hoping you get improved success ASAP.

FWIW my sleep doc has prescribed a new PSG (with my CPAP), blood tests (thyroid, iron, ferritin), echocardiogram and pulmonary function test. He didn't rule out CPAP discontinuance if my heart checks out.

Don't forget to add testosterone to the testing.
