Tried again last night with the backpack to keep me on my side and also using the soft cervical collar. Had a lot more sp02 drops than the previous night this time. Very annoying to say the least, haven't changed any variables but I realise it could even be effected by how my head is resting on the pillow.

I have the same experience with the head position.
Mouth taping can further reduce your OAs; you can try increasing the minimum pressure to 14.4 cm.

Hello again. I took some time and made some changes and ran with them for a few nights. I swapped from my full face mask back to a nasal pillow and started using mouth tape, the full mask was causing a lot of irritation under my nose and beginning to really hurt. Changing this didn't effect my numbers really.
What did seem to help was changing to a soft down pillow, I got my lowest AHI ever last night which is great. On the downside I'm still having some considerable desaturations it seems which I believe is caused by kinking of my neck still. I woke up last night to my spo2 sensor warning me of a desaturation around 07:25 and I noticed my neck was a little bit bent towards my chest while laying on my side, I've noticed this happening before. I use a cervical collar (12cm 3m futuro), I can't seem to find anything taller to experiment with. While the numbers are generally good I'm still not feeling good, I think the desaturations are causing significant arousals that keep me from feeling rested. Maybe this can be solved with a pressure increase?

Velpeau has a whole series of collars with different heights, including 4". On their homepage, you can find detailed size dimensions for each. I ordered four different sizes to find the right one. The silicon one lasts forever, although it is a bit rigid. There is a significant price drop right now,

Thanks for the suggestion. Seems like Velpeau are hard to purchase here in Europe. I had not considered any of the non foam ones yet but surely I can find something here.
