UARS - Wake up after 4-6 hours and take mask off

UARS - Wake up after 4-6 hours and take mask off
Background: So after about a month of tweaking CPAP settings and trying a bunch of different masks I've finally gotten to the point where my setup is comfortable. I'm still tweaking the straps to balance comfort and leak rate so still getting a few leaks. I have a suspected UARS diagnosis.

So I'm getting roughly 4-6 hours of sleep before I wake up (not really sure why). I normally take my mask off when I wake up because its hard to get back to sleep again with it on. I think I avoid trying harder to keep it on when I wake up because I don't want to sit lying awake for hours and then feel worse overall the next day.

Overall though I seem to gradually be feeling less tired for longer during the day now so I'm very happy about that!

Just wondering if its something that's likely to improve with time or if there is anything obvious from my charts that can be done to try and improve my therapy?

RE: UARS - Wake up after 4-6 hours and take mask off
Your therapy with Vauto has eliminated any potential UARS issues, and I don't see any element of your charts that relate to flow limitation or upper airway flow restriction. There are some small arousals during the night, but whether there is any nexus to a respiratory issue, or if it just a movement at night is not obvious from the charts. Therapy looks very good and effective from an AHI point of view, however there is room for improvement with leaks.

Night time awakenings are a difficult issue to tackle. You have concluded that it is harder to go back to sleep with the mask on than to just take it off, and this has become a behavioral pattern. I can't say why a mask should make it any harder to resume sleep, especially if you experience apnea or airway restriction without it. You might want to consider a mild sleep aid like time-release melatonin to help maintain sleep. It's encouraging that your therapy is successful from your perspective of how you feel, and that may be the most important point.

For improvements, look for ways to prevent arousals and break the behavioral habit of removing the mask upon awakening. It's easy to form a self-fulfilling conclusion that it is the therapy that prevents resumption of sleep, but this may be overcome by behavior modification, substituting that with an attitude that the therapy is relaxing and makes sleep easier. Other than that, the leaks appear to be the most disruptive factor affecting your sleep.
