Worried about RERAs
Hi,

So turns out I had been missing information from looking at my CPAP graphs. Because I was reading daily aggregates from the reports, I didn't see the information about RERAs I've discovered now by looking at a specific day.

Lately I don't feel I'm sleeping well, I have plenty of arousals and having a look at the daily data in the ResAssist Cloud App from Resvent I've seen I'm on RERA most (if not all) my sleep.

I've included the last three days, but going back in time, most days look like the last two days shown here. I know it's not the detailed info one can get from OSCAR, but my machine is not supported. I'd appreciate any light you guys can throw on this.

Just in case it helps, here's an extract of the sleep study I did a year ago. I had to translate it to English, so some terms may not be fully accurate:

Sleep structure:
Time in bed: 574,2 min.
Sleep latency: 5,8 min.
REM phase latency: 86,5 min.
Actual sleep total time: 329,5 min
Arousals from 1 to 5 min: 14
Arousals >5 min: 6
WASO: 142,0
Sleep efficiency: 57,4%.

Sleep phases:
NREM1: 21,5% (71,0 min).
NREM2: 58,1% (191,5 min).
NREM3: 8,2% (27,0 min).
REM: 12,1% (40,0 min).
Total time awake: 30,1% (245,0 min).
Arousals no.: 63
Spontaneous Arousals: 9
Total arousals index: 11,5
RERA: -

Respiratory parameters:
AHl: 9,3
OSA: 1,5
Mixed apneas: -
Central apneas: -
Hypoapneas: 7,8
Apneas/hypoapnea (supine decubitus): 14,9
Number of oxygen desaturations: 9
Min oxygen saturation recorded: 90,6%
Average oxygen saturation: 95,5%.

Not that familiar with the iBreeze. Can you tell us if this is an auto Cpap or a cpap?

It looks like you're holding steady at a pressure of 5. With Hypopnea, Flow Limitation and RERAs, it might be beneficial to use the IPR feature. I'll assume it's similar to ResMed's EPR.

Also, you could probably use a little higher pressure to help with the snores.

If you're willing, try a pressure of 7. Set your IPR to 2. Let's see how that works for a couple days.

Thanks for your quick answer!

The iBreeze C20 is just a regular CPAP, no Auto function. I was using a pressure of 5 with IPR of 3, that I'm reading is probably not the best setting as the minimum documented pressure of the machine is 3.

I've already set up the machine for a pressure of 7, IPR=2 and a ramp of 10 min. I'll try for a a couple of nights and report back with the results.

Note: The IPR setting shows "Off" in the screenshots you posted above.

Yeah, about the IPR showing as OFF I noticed it too after you asked about enabling it, looks like a bug of the platform not showing up the actual setting.

I've rechecked and it's enabled in the machine. Just to be sure, I've had a look at short time intervals in the pressure graphs and I can see the pressure going down to 3 and up to 5, which is what I'd expect.

The Resmed offers a "full time" and "ramp only" option for their equivalent to this feature. Does yours have anything like this?

It doesn't look like it has it. When I adjust iPR, there's only a slider to adjust the pressure difference from 0 to 3, and that's it.

About the ramp, there's P Ramp, to adjust ramp starting pressure, and T Ramp, to adjust ramp duration in minutes. Here's the progression corresponding to the last day of data I sent above where both the iPR pressure variation and the ramp can be seen. So iPR will apply full time (including ramp), or won't apply at all.
