heyhey's - Therapy Thread

I also thought an asv would fix your airflow limit.

I need help for "Asv settings"
Hey I am still tired on Asv and my Oscar says almost 0 Ahi, but I am curious if it can have something to do with my settings, which affect my flow rate which make me tired still, or whatever it might be, so I would like if somebody would look into my settings, and come with some adjustment that might help me.

By lining up your right hand side charts vertically (events at top) (and mask leaks), it looks like a quick, large leak causes each of the disordered breathing patterns you show close ups of.  You could have simply rolled over and changed positions with caused the mask leak.  You recovered after that because the ASV increased pressure accordingly.

    See if decreasing leaks stops this.  You can always upload more OSCAR charts.

I need help for my Asv settings
Hi I know it says 0 ahi, but I am still tired, I don't know if the epap, ps mim/max adjustment might help? maybe the Ipap isn't correct, so I was hoping if someone could come with a suggestion to my settings, by looking at this picture.

What are you treating, and how did you arrive at such high PS min and PS max? Typical would be PS min 3.0 and PS max 15.0. I have merged your three threads and hopefully you will get more responses to your question and we get this sorted out. I would like to suggest some settings to try, and we can take a look at your results and decide the best way to move forward.

Mode: ASVauto
EPAP min: 5.0
EPAP max: 8.0
PS min: 3.0
PS max 12.0

Need some help for my Asv settings
I need help for my Asv settings

Hi I know it says 0 ahi, but I am still tired, I don't know if the epap, ps mim/max adjustment might help? maybe the Ipap isn't correct, so I was hoping if someone could come with a suggestion to my settings, by looking at this picture.

hey I am not sure how they ended up so high, but I have tried your suggestions, and I link 2 pictures with it, and a third with a littler higher max epap.

I don't get why have 8 max epap, when I see on the charts that it wants to go above that? like on the third picture, it goes up above 10 max epap at sometimes, is there a reason it shouldn't get allowed that last amount of air? 

so what do you think of it, and should I try and adjust anything?

It can't go above EPAP Max 10 with the EPAP range at min 5 Max 8. It doesn't work like that.

However, it does increase pressures above EPAP Max 8 because PS has it going up, adding it to the IPAP. It is not hitting your Max IPAP in any of these. It's typically 2 below Max IPAP 20, so it had room to go up if needed, but it does not need to.

By the numbers, your therapy is showing treated.

Here's your pressure settings with the latter 2:
EPAP 5-8
PS 3-12
Gives IPAP of 8-20 because IPAP is EPAP plus PS of that moment.

You had EPAP 6-10.6 January 27, which had the most disruptions at 4 sessions that night.

It is not bumping your Max pressures on any of these charts, it's not hindered.

Something is disrupting your sessions, what I'm not sure. Of these 3 charts, you had 4, then 3, then 2 session starts respective dates.

So I'm thinking there's a discomfort issue. Tell us what that is.

Edit: from post 1, you mention something about airflow limit. What do you mean by that? And what was the reason for ASV for you?

Should I change from ASv to VAuto BiPAP?
Well when I look at my charts I am not sure if my flow flow Rate or eles is current? if it's Rera or Uars I have, can anybody see that on my charts?

So might Vauto Bipap be more suited for me instead of asv, as Asv isn't the best for Reras? ? I didn't have that many centrals before, but my flow Rate seems kind of off often when I sleep. 

I still feel tired after I use my Asv, now I am curious if Bipap vauto is better suited for me?

You are using some pretty novel settings in ASVauto with EPAP min 5.8, EPAP max 13.0 and PS 5.0 to 15.0 in the November graphs and EPAP min 4.0, EPAP max 9.0? and PS 2.4 to 12.0 in May. If you're not treating central or mixed apnea, there is no good reason to be using ASV, and it appears to be resulting in some abnormal paced respiration based on the November 24 graph. The May 10 graph has a lot of settings and statistics blanked out, but the period selected shows very high pressure support, and if you're not treating centrals, the Vauto would deliver a fixed pressure support that would probably be more effective for obstructive apnea and flow limitation. These graphs are not very useful to know why you are using these settings or to suggest what might improve your results and sleep quality.
