myratan therapy help

myratan therapy help
hello. newish user of cpap here.

I am not sure where to go next. My sleep tech set me up on 5-15 which you can see an example of attached. Regardless of pressure, I spend at least 2 hours before I wake up having arousals every 30 seconds to 2 minutes, usually when I'm dreaming as I will wake up fully many many times. This also happens at a pressure of 18-20 which I'll attach next. I seem to have an arousal after a bunch of flow limitations? My tidal volume also goes down before an arousal, sometimes into the 200s.

Background about me:
Female, 163cm, 65kg.
Mild OSA, severe during REM and "possible nocturnal hypoventilation" which hasn't been further investigated yet.
Have 2 neurological chronic illlnesses, 1 connective tissue disease
My illnesses can cause respiratory muscle weakness although this has not been confirmed for me yet.
Low arousal threshold - I wake up to any and everything

I barely slept during my level 2 home sleep study so I'm repeating it next month.

Any help on where to go next would be appreciated. Thanks Smile

Myra

Charts at 18-20 added. Same problem. My thoughts are I need to try a bilevel?

RE: myratan therapy help
On November 23, your pressure settings were 18 to 20 with EPR 3 and on January 3, you had dropped to pressure from 5 to 15 with EPR 3. Your therapy is generally good with some minor CA events and consistent with UARS a pretty steady flow limitation that appears to result in fairly frequent arousals. I think your ideal pressures are 7 minimum 10 maximum with EPR 3. I'm sorry your post sat without a response so long, but I have to run this morning, so give this a try and I'll check back.

You're in the best of hands with Sleeprider.

I see that your median respiration rate is somewhat below normal, but your median tidal volume looks good, and your median minute ventilation seems normal for a person with a small build. (Tidal volume is the amount of air you move during a single breath; minute ventilation is the amount of air you move per minute.) My take-away is that you breath somewhat slowly but fairly deeply. I hope Sleeprider will weigh in on that.

You might want to invest in an Oscar-compatible O2 monitor, just to see how your O2 levels behave during then night. You can check the list of monitors be clicking on "Oximetry Wizard" on the Oscar welcome page.

Still waiting for a granddaughter to be born, but I had to look after my daughter's house and puppy menagerie this morning. Anyway, give these setting a try and you won't be ignored when you post back.

Thank you Smile I will try the more narrow range and let you know how it goes.

No worries about the delay. This is a very busy place!

I actually already have a Wellue O2 ring! My o2 is usually fine, a few drops during rem I'm guessing. My heart rate is absolutely insane though and matches the arousals in the flow rate. I added a couple of examples, one at 5-15 and one at 18-20.
