I generally try to keep the collar tight, but I can pay extra attention to it.
It makes me wonder, though, why I go through periods of the night when there are fewer issues.
I believe I mostly sleep on my back.

When your info is scattered across 7 threads and I'm on my phone it is not easy, but I'm back.

Where are you with pillows?
Try and minimize your pillow. Anything that lifts your head could be the problem. Some professional studies that are underway and studying this chin tuck refer to it as cervical (neck) alignment. Think in those terms.

Think about taking a video of yourself and see what you can see at or around the time of this first hour cluster

Will do on the pillow. I have an apnea pillow with cutouts, but it is rather high.

Same results last night... A burst of AHI up front, and one other time during the night.

I changed the EPR to 3, with a min and max pressure of 9 and 15.

I also slept on a towel folded over for a pillow, and used my cervical collar.

Should I increase the min pressure, perhaps to avoid getting the early burst?

Here is my chart for my continuing, mysterious problem of high AHIs at the beginning of sleep.

Last night, I used a towel for a pillow, EPR = 3, cervical collars... but the same results.

I think I figured something out. Last night I went back to the tennis-ball-sewed-in-the-T-shirt-trick to keep myself on my side.

Looks like the way forward.
