Do I have to readjust CPAP settings after losing weight? - hshamci - 12-08-2022

Hi,
I have been using CPAP for almost three years now
Settings to my machine:
ResMed
AirSense 10 AutoSet
Min 6.0 Max 16.0

I didn't have any problems so far (apart from some leaks every now and then) and my AHI was almost always below 2. 
The few months I have lost some weight, 15% of my total weight, and my AHI has increased slightly to 3-4. I am waking up really tired.

I am wondering if I need to adjust CPAP settings after my weight loss

Two things are going on. Your pressure is very low and with EPR the minimum exhale pressure is less than 4.0 cm which means the CPAP will not respond properly when pressure increase is needed. The lowest pressure the machine can produce is 4.0 cm, so with a minimum pressure of 6.0, the starting pressure is 6.0/4.0, and the first increase in pressure only achieves 7.0/4.0 before exhale pressure starts to rise. A minimum pressure with EPR 3 should be 7.0 cm. Secondly, you have clusters of obstructive apnea. This may not be related to weight loss, but to the use of extra pillows or a sleep position that encourages chin-tucking which obstructs the airway.

In my opinion you need to increase your pressure to minimum 7.0, maximum 14.0 EPR 3 and evaluate your sleep position for possible positional apnea. http://www.apneaboard.com/wiki/index.php/Optimizing_therapy#Positional_Apnea I'm not saying you need a soft cervical collar as this may resolve by being aware of it.

Let's take a look at the results tomorrow and we can decide on any further changes. There is not much we can do about CA events without cutting back on EPR which would likely increase flow limits, so be sure to include the flow limit chart with the next post.

Looks much better. I think a minimum pressure at 8.0 is the only change I would suggest at this point, but the machine is getting into the effective zone much faster. How was comfort?

It was actually comfort but I wake up many times in the night

Thanks so much for your help.
Attached the charts for 09.12.22 with AHI: 1.58 and 10.12.22, AHI: 1,76
08.12.22 AHI was 1,63

Usually when AHI<1 , it's very rare, I'm very active on this day and feel like I'm in better shape so I'm wondering if there are other settings that can be adjusted.
I know many who have AHI<0.5 most of the days

Your obstructive apnea index is between .5 and 1.0 with these settings, and there are some random CA events that are keeping AHI at 1.5-2.0. I think it's far more important that you feel active and rested, than we try to chase these numbers. I recommend continuing with these settings for a while and see how a longer-term trend of your therapy settles in. It seems clear you don't need pressures above 11-12 cm so if you want to cap maximum pressure at a lower level, that is fine, but otherwise, there is nothing I want to see changed at this time.
